EE removed heater core from Skidsteer loader. Due to location, EE had hard hat off to remove. EE took the heater core to parts washer and laid it on top of washer, before putting hard hat on. EE bent over to pick up strap and heater core fell and hit top of head. Laceration required 3 staples.

Showing all 3EE was cleaning up on Scalping tower when a rock came off screen and struck EE on Back. Doctor said the EE's back was bruised. Came back to work. On 4/21/09, EE went back to doctor for checkup and doctor scheduled physical therapy for bruise. Started physical therapy (ten sessions) on 4/27/09, and accident became reportable on 4/27/2009.
The employee was cleaning out a clogged chute leading off the primary crusher. Once unclogged, the employee walked away from the chute and rock fell from the top of the chute, striking the employee's left hand.
