OPERATOR GOT OUT OF BACK HOE TO CHECK HIS FUEL LEVEL HE DID NOT HAVE A FLASHLIGHT SO HE USED HIS LIGHTER FUMES FROM THE FUEL TANK IGNITED FLASHED CAUSING BURNS TO HIS FACE AND SHOULDER
