EMPLOYEE WAS WORKING OUTSIDE OF D3 MILL BURNING LINER BOLTS OFF WHEN A RUBBER WASHER CAUGHT FIRE, POPPED, AND RUBBER FLEW INTO HIS LEFT EYE. HE WAS WEARING SAFETY AND CUTTING GLASSES AT THE TI ME OF THE ACCIDENT.
