EMPLOYEE WAS LIFTING A ROCK AND SLIPPED AND HURT HIS LOWER BACK. THIS ACCIDENT WAS NOT REPORTED UNTIL THE STATE CALLED THIS OFFICE THEN THE EMPLOYEE CALLED US.
