EE was replacing tram-cylinder on HP4 cone and got EE's thumb pinched between the cylinder and cone, resulting in laceration/partial amputation of the left thumb.

Showing all 18EE was welding, and when EE finished with the rod, threw it from the holder. The rod fell into EE's boot and burned EE.
Employee was performing maintenance on 870 transmission hydraulic pump. While positioning pump on service truck bed, pump began to roll to employee's right. Employee attempted to stop pump from rolling which resulted in employee's right ring finger being pinched between pump and vise mounted to service truck bed. Employee required 15 stitches to right ring finger.
Ee was working on belt. the need was to remove a rock slab from the belt. ee was accompanied by another employee. in process of removing the slab, the slab slipped and landed on ee's left wrist. ee immediatley notified about the incident to plant manager. ee was taken to urgent care location where X-Ray were performed. the X-ray reports came back negative. ee on light duty
While positioning a sheet of 5'x13' 1/4" plate steel to weld, the injured employee became momentarily trapped underneath it when the steel came off of the hook used to lift it into position and fell on top of EE.
Miner scheduled eye doctor appointment after complaining of eye irritation. Finished shift and went to doctors following day. Piece of steel was found in eye.
Employee was disconnecting 4 inch coupling, as employee tried to pull it apart. The coupling struck employee in the mouth. chipping employee's tooth.
Two employees including one supervisor was in the Quarry tunnel attempting to raise the gate for the tail pulley. After repeated attempts, the wall went up and jammed one employee's right hand between the pulley and the wall of the tunnel. It took about 3 minutes to pry their hand out from being squeezed.
EE and two others were working in the Tunnel in the Quarry trying to raid the gate for the tail-pulley. When it became unstuck, EE's hand got stuck between the gate and the wall. The other two rushed to help free EE's hand using a piece of 2x4 wood to pry EE's hand out. It took about 3 minutes to free EE's right hand; EE's thumb was numb.
While installing an engine on a Caterpillar 992G machine, our Field Service Technician was under the machine installing the drive shaft and hydraulic lines. While attempting to move from under the machine, our Technician hit hard hat on the step located on the side of the machine knocking off hard hat resulting in a scalp laceration requiring stitches.
Employee was replacing a roller that goes underneath the conveyor belt #7 when employee tried to use force and pushed to flip the roller in place it slipped and became in contact with the employee's right middle finger.

Employee had just finished his shift when he bent over to pick up his lunch cooler and strained his back.
Employee was changing screen panels on the Telsmith #2 middle deck. When he rolled over to reach for a panel he felt pain in his chest area. He was taken to the hospital and diagnosed with a pulled chest muscle.
Employee was gripping catwalk railing to step up to steel catwalk, somehow he twisted his right arm and he felt something "pop" in his elbow. He was in alot of pain and on 10/06/2011 he was diagnosed with a bicep tendon rupture.
Smashed end of finger between cylinder head and pin shoulder.
Two steel plates approx. 2'x 4' were set against each other upright near the base of the crusher, when EE was putting a clevis on one plate, they fell over his leg. EE's leg was broken from the weight of the plates.
3 people were carrying heater down stairs from little shaker. (Successfully) loaded the heater in back of truck, everyone left, 10 to 15 min later EE called his supervisor & reported his back was hurting.
