The fuel handling Tabor Screen reject chute contains a door for access and cleaning. While removing the chute door, the door slid down toward the floor. The operator grabbed the door with his left hand and felt a pain in his shoulder area. Medical treatment was provided which included a prescription and scheduled physical therapy.

Employee was changing a fuel filter on a pump. Employee grabbed the fuel filter with his right hand and the fuel line with his left hand to remove the fuel filter. While twisting and pulling the fuel filter the fuel line released suddenly and his left hand hit the frame of the pump. Employee's left third finger was cut on the outside of the first knuckle and required five stitches.

EE was assigned to calibrate the B Ramsey belt scale load cell. The load cell includes a pivot arm which fluctuates up and down. As the arm is displaced, a signal is transmitted to a meter where the weight of the waste coal is determined. The arm is manually lifted to relieve any tension. EE improperly lifted the pivot arm while using only his right arm and improper body position.

EE was working on a BMR(crusher) liner bolt replacement. Crew was placing the top lid on the BMR. 2 cranes/hoists are used to move the lid. The first positions the lid approx 3' short of its final placemen. The lid was placed on top of the BMR while the second chain hoist was rigged. While resting on the machine, the lid teetered and fell approx 18" on a mechanics foot.
