Miner was skipping a stoper drill from the 3rd level to the stope level above. A rock was dislodged from the stope floor and fell down the raise, striking the miner in the face, breaking his safety glasses and injuring his right eye.

Showing all 3Miner was training underground when co-worker's boots flung mud into miner's eye. Miner was not wearing proper eye-protection. Miner incurred no injury or damage to eye however he did receive prescription medication for pain and antibiotic.
While checking/replacing the mine phone in the mine office, two dogs began to fight in the office. Victim and co-worker attempted to break up the dogs and remove them from the premises. As victim kicked at dogs, the co-worker swung a mine phone at the dogs, connecting with victim's leg. A minor laceration to victim's leg, requiring four (4) sutures resulted.
