While crossing the inby airlock door, someone attempted to open the outby airlock door causing an increase in air pressure on the inby airlock door. In doing so, the 2nd door of the inby set of doors tried to close and struck the employee on the right arm.

Showing all 18Employee claims to have been loading a take up jack into a scoop bucket and felt pain in his right shoulder. Note: Company feels an accident did not occur. He left work after filling out the accident report himself and did not contact the company until 6-21-10. Asked is he had seen a doctor, he replied NO. During Part 50 Audit it was discovered he had.
While supplying the roof bolter with roof bolt plates a piece of wire came in contact with the EE's right eye. The doctor "prescribed medication for eye irritation" for 5 days. Discovered during a Part 50 Audit.
EE was moving two 13" tires from the back of the mantrip to the front deck of the mantrip. When he placed the tires on the deck he felt pain in his lower back. The EE went to Med Express where he was examined by a Physician Assistant. EE was seen by a Doctor on 3/22/2010.
Loading new plastic waterline onto a scoop EE felt pain in his lower back. The EE went to the doctor and did not inform management that he had went to a doctor and or had restrictions. Discovered due to Part 50 Audit.
EE was installing a connecting pin in the cable sled toe plate when he dropped the pin and pinched his finger between the lip of the pin and the sled toe plate.
While filling the rock dusting maachine with rock dust, Injured complained with pain to his groin area.

EE was working on a 980 Cat Endloader brakes. He took off the master cylinder top, his hand slipped, cutting his right ring finger knuckle. The cut started getting infected, he went ot the doctor on 5/21/08. No stitches.
Employee was pulling choker cable on miner to hook up pull rope. Metal wire in choker cable went into left hand cutting it.
EE was pulling a piece of rock off top of Joy miner when his hand slipped and was cut on edge of rock.
EE fell while carring a bucket of plaster. which had the lid off of it. The brown liquid on top of the plaster splashed up into his lt eye.
EE was hanging miner cable with nylon rope hanger with metal hook on one end. The cable slipped out of his hands pulling the metal hook into his rt hand.
Butch ferrell cut the plam of his lt hand while carring metal panels for an overcast.
Employee had his right foot on belt while belt was being laped up. Belt clamp came off belt and hit him on right leg above ankle.
Employee suffered small cut to right eye. When slamp he was working on slipped from his hand hitting a metal band and causing a piece of metal to break off and enter eye. Not reported to this office until 7/23/07.
Employee and co-worker were pitching cribs up on a rock fall. Employee pulled something in stomach.
