EE HAD FINISHED WASHING DUMP TRUCK WINDSHIELD AND WAS CARRYING A 5# BUCKET, 3/4 FULL, BACK TO WASH RACK. WHEN HE BENT DOWN TO SET THE BUCKET DOWN, HE FELT A SHARP PAIN TO LEFT LOWER BACK, STATES PAIN IS A 9 ON 1-10 PAIN SCALE, NO RADIATION OF PAIN. EE WAS GIVEN PAIN MEDICATION VIA HYPODERMIC NEEDLE INJECTION. EE RETURNED TO REGULAR DUTY THE SAME DAY.

EE DENIES PAIN OR DISCOMFORT TO ANY OTHER PART OF BODY EXCEPT FOR LOW BACK. ON 07/17/07, EE'S TREATING PHYSICIAN TOOK HIM OFF WORK. EE CURRENTLY LOSING WORK DAYS.
EE stated that he was operating a valve when he felt a pain in his lower right abdominal region. Diagnosis is a rib separation. Employee lost one day of work due to nausea after taking ibuprofen for pain relief. EE returned to to work and was placed on restricted duty on 5/14/07.
EE sustained minor right eye irritation when he was contacted by dilute liquor solution. Plant physician administered a one time application of a prescription medication. EE on restricted duty as of 05/15/07.
EE was operating a valve when he felt a "snap" in his right elbow. On 04/04/07, EE was placed on restricted duty.
EE was working a valve handle back and forth when he contacted a sharp edge on the handle with his left forearm. Result was a laceration that required five sutures to repair. No lost work days or restricted duty.
EE failed to notice that a valve was opened and he opened the valve allowing hot caustic slurry to come out and contact his left forearm. EE sustained second degree burns on his left forearm. Restricted duty only, no lost work days.
EE struck his left middle finger against a walkway handrail while working on some conduit. Diagnosed as a fractured finger on 02/16/07.
EE was attempting to clear a caustic line. An open drain valve was nearby. When the line cleared, caustic came out the drain line and sprayed onto EE's face and abdomen. Restricted duty, office work only, EE lost four work days.
EE was moving a wheeled work stand from one location to another. He dropped one side of the work stand on his right foot causing a contusion and swelling. The EE could not wear shoes due to the swelling and was not able to report to work. Out of the plant on 01/08/07.
EE felt a pain in his lower back when he pushed on a hoist. Taken off work by his treating physician on 01/09/07.
EE felt a pain in his groin area while opening a six inch plug valve. On 01/08/07, EE was taken off work by his treating physician. Diagnosed with an inguinal hernia. Surgery is scheduled for 01/17/07.
EE was pulling a 1 inch hose toward himself. Diluted caustic in the hose escaped and splashed onto the EE's face and right eye. EE was given prescription medication and the eye was bandaged. EE placed on restricted duty on 12/18/06
Employee got caustic inside boot causing 2nd & 3rd degree burns to foot.
EE was assigned the task of emptying material out of 50 lbs bags into a refractory mixer. The next day EE complaining of pain in both shoulders. On 02/12/2007, plant doctor gave the EE a Prednisone Taper prescription.
EE was cleaning out a screen box on a pump. As EE pulled the screen out of the box, the screen fell downward and caught EE's right ring finger between the screen and the screen box. EE sustained a minor fracture on his right ring finger. EE placed on restricted duty.
EE was picking up hydrate scale from the ground. As he picked up a piece of scale his right hand middle finger got caught between the scale and an angle iron. The EE's upward hand motion resulted in a fracture of the right hand middle finger. No lost work days, restricted duty only.
EE sustained a fractured finger in his right thumb when it was caught between a lifting hook and a lift cable lifting eye. EE had been attempting to hang the lifting eye on the lifting hook when the incident occurred. EE did not lose work days. EE is on restricted duty.
EE was placing a shaft in a bench vise. The shaft slipped and caught the EE's left hand between the shaft and the vise. Result was a fractured finger in his left hand. No lost workdays, restricted duty only.
EE opened a flanged bottom plate on a tank that had approximately 214,000 gallons of a caustic solution. EE sustaine chemical and thermal burns to both eyes, both arms and chest.EE began losing work days on 09/11/2006.
On 08/10/06, EE sustained a minor abrasion on his left knee while ascending onto a three foot workstand. On 12/19/06, EE visited the plant medical facility and was experiencing left knee pain. Plant doctor administered a mini-steriod injection to his left knee for the pain. No lost work days or restricted duty. EE returned to work on the same day.
EE's eyes were sprayed with a dilute caustic solution when the one inch hose he was using developed a leak. EE was treated with prescription strength medications and placed on restricted duty.
EE felt a pain in his lower back after hooking up a welding machine to a three wheel scooter. On 8/11/2006, EE was taken off work by his treating physician.
EE was removing an inspection blind on a caustic process line that had been depressurized and drained. When the EE opened the inspection blind residual caustic dripped onto both of his thighs causing second degree burns on both thighs. Doctor took EE off work on 07/07/2006.
EE was setting the top heater cover on a vertical heater. His left middle finger got caught between the cover and the heater body resulting in a fractured left middle finger. No lost work days, restricted duty only.
EE had poured a liquid sample into a glass test tube and was in the process of inserting a cap onto the test tube. When the EE applied pressure to seal the cap, the glass test tube broke causing a laceration on the EE's left hand index finger. The laceration required sutures to repair. EE was in the process of closing an inspection port on a gas suspension calciner when a puff of hot alumina contacted the right side of EE's face. EE sustained second degree burns on his right ear. No lost work days, EE placed on restricted duty.
EE was opening a 50lb box of welding rods, the metal ribbon on the opening tore a one angle and slid across his left arm. The result was a 4' long cut that required stitches.

EE's right hand got caught between a metal rod and a conveyor housing. This resulted in a laceration that required sutures to repair.
EE experienced lower back pain after he moved a rail car door opener. He was taken to a local hospital for evaluation. The Emergency Room doctor placed employee on sedentary work only restrictions.
EE attempted to move a portable metal stand that were stacked on top of each other. As the EE moved one of the stands he felt a pain in his lower back. Plant doctor put EE on restricted duty.

EE states that on 08/27/05 he got some caustic on his left leg. On 09/02/2005, after his four regularly scheduled off days, he reported to his supervisor that he had a caustic burn on his left leg. The burn had become infected and the plant physician prescribed an antibiotic for treatment of the burn.
EE was closing a valve when he felt a pain in his left shoulder. EE had been scheduled for shoulder surgery on 12/16/05, EE and treating physician changed the surgery date to 12/14/05.
EE lifted a five gallon plastic bucket that was full of repair parts. While at home after work, EE started feeling pain in his lower back. On 08/10/05, the plant doctor prescribed a muscle relaxant and pain medication. EE placed on work restrictions.
EE was disposing of trash that he had collected during his housekeeping duties. As he twisted to put the trash into a trash can, he felt a pain in his lower back. Plant doctor issued prescription strength muscle relaxant. No lost time and no restrictions.
EE was rodding out a drain valve when a dilute caustic/slurry solution splashed out of the drain valve and onto his abdomen. EE sustained two small 3rd degree burns in his abdominal region.
EE was replacing a pipe fitting when a hammer that was on an overhead pipe fell and splashed dilute caustic into the EE's left eye. Result was a corneal chemical burn that required prescription medication. EE was taken off of work by his treating physician on 06/18/05.
EE was hosing down with a dilute caustic solution. EE felt an itching sensation in his right foot. EE did not report the incident to his supervisor until 6/03. EE went home and discovered that he had a small chemical burn area in his right foot. The affected area became infected due to a lack of prompt medical treatment and poor personal hygiene.
EE was rolling up a fifty foot section of one inch air hose that was no longer in service. In the process, the employee twisted his right knee. On May 24, 2005, the plant physician placed him on restricted duty.
The equipment operator was working in a digester tank. He took the machine to the bottom of the tank to make a repair. When climbing out of the work basket, his lifeline became disconnected. It retracted quickly striking the hole watch in the face.
Maintenance Mechanic was attempting to flange up a section of pipe to a vessel. His left hand, fourth finger got caught between two flanges, resulting in a small laceration that required sutures to repair. Prescription strength pain medication was also administered. No restrictions or lost time.
EE struck his elbow against some adjacent piping while operating a valve. On 04/05/2005, EE was placed on restricted duty in his work area.
EE was picking up a conveyor belt clamp. The clamp slipped and EE's right wrist "popped". On 03/22/2005, EE was given an injection to his right wrist. EE was also placed on restricted duty.
