Employee stated that while installing I-beams along the #4B Belt he was struck on the head/back by a beam when the employee holding the beam lost his balance and dropped the beam. Employee began missing work, complained of back pain, saw a doctor on 01/31/13, and medical received on 2/7/13 stated "mild depression of the T5 & T8 vertebral bodies, with no acute fractures seen".

The employee explained he reached over and picked up a roof bolt and pie pan out of the roadway and pain shot down his hip and left leg. The initial WC BI-1 shows prior surgery prior injuries on his lumbar spine and not acute injury.

The employee claimed pain to his back hooking a duster to a tractor 11/15/2012. He continued to work until November 26th. He has been seeing a medical Doctor for other complications of the same body parts (chronic in nature) history of lumbar disk.

The employee injured a permanent tooth by being struck in the mouth by a chain. He pulled on the to free it from supplies causing the chain end to flip striking his mouth.

The employee strained his lower back picking up a 5 gal container of water to remove from a pressure line he was working on.

The employee was loading roof bolts onto the storage tray for the Fletcher RR II and ruptured his groin. The event has resulted in a hernia lost time injury.

The employee contacted a sharp object near the exhaust system on the #766 diesel vehicle during welding. The incident resulted in a laceration requiring sutures.

The employee stated he caught his right index finger between the cable and the bolt plate on pizza pan for rib support. The incident resulted in a medical reportable cut/sutured injury.

The employee stated he was moving rock off miner and pinched his finger (left middle) between rock and top. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee stated he was pushing and twisting on roof bolt for installation and injured a mid section vertebrate. He is missing work as a direct result of this event.

The employee stated he strained his back lifting on rock at the 3A belt head while cleaning the area. The incident has resulted in lost time.

The employee opening an airlock door felt burning and pulling in his lower back. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV.

The employee hanging miner cable by himself stated he felt a burning sensation from the middle back up to including his neck. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV". 8/7/2012 this claim was denied (no medical evidence).

The employee alleged he strained his back bending a roof bolt on 4 section during the non-producing owl shift. Medical received on 05/14/2012 states spondylosis and lumbar lordosis which suggest presence of an illness rather than an acute process.

The employee stated he was supplying the RBM in the face of no. 2. He was transferring bolts from the scoop bucket onto the bolting machine when his right hand slipped causing the bolts to his left hand to be between the bolts and the scoop frame.

The employee reported he was lifting Omega block and wrenched his neck when standing up contacting roof with hat. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was lifting and placing shield over shuttle car reel and claimed lower back injury.

The employee claims he suffered a back strain lifting miner cable hanging it. He is missing work, and the company has not received any medical as of 10/24/11. The incident was charged "No action taken" initially.

The employee was struck on top of left foot by a rub rail on the continuous miner upon removal of hold hinge pin. The incident resulted in a Fx metatarsal bone.

The employee reports he injured his lower back disconnecting a winch rope from a trash bin. He claims he was standing in mud and flipped the rope hook backwards creating a lost time injury. His medical suggest he has history of same said problems reported in the past.

The employee stated he grabbed and pulled down on a string causing a spad nail that was attached to to strike his thumb. The incident resulted in a lost time injury (fracture).

The employee fractured his left ankle dismounting a mantrip on the surface ending his shift. The incident resulted in a lost time injury.

EE was loading timbers along 30 roadway. EE eas laying a timber out to cut and it slipped and smashed index finger between timber and block left finger.

The employee stated his left thumb made contact with a sharp object. He continued to work until April 9th and start missing work due to infection. Also the mine was informed on April 19th the injury required sutures on April 2nd. The injury has resulted in a lost time case.

The employee was lifting and pulling on a water pump mounted on maintenance diesel vehicle. He is diagnosed with rotor cuff damage.

The employee stated he was loading a 13 HP de-watering pump into a diesel vehicle and injured his finger between the pump and vehicle frame. The injury resulted in a medical reportable event. He finger required sutures.

Pulling welder cable and heard something pop in shoulder. Did not receive report until 4-1-11.

The employee caught his right little finger between the scoop frame and a crib block. He was attempting to remove the wooden crib block from the scoop bucket. The event resulted in a lost time injury.

Working on RB Boom and got (L) arm caught in drill pot of machine.

The employee was removing stack steel that was stuck in the mine roof. One of the pieces of steel fell out, striking his left hand, causing a laceration to his ring finger that required 8-sutures.

Employee was pulling cables. He backed into a roof strap and cut back.

The ee was experiencing pain in neck and elbow when handling miner cable 4-section #3 entry CC outby face in intersection on 11/30/10. Medical received 12/8/10 shows there was no acute injuries of neck, shoulder, arm or elbow. However other medical shows other issues exist causing pain to the affected areas.

Twisted left ankle while loading roof bolts supplies.

The employee was loading empty wooden pallets onto a supply car and reported he strained his lower back. He started missing work on October 20, 2010.

The employee was removing ventilation curtain from feeder and cut his finger. The event resulted in a reportable medical. He received sutures.

EE stated he grabbed a handful of bolts and twisted around to put them on the tray. While twisting he says he hurt his back. *EE did not report this injury until 5/17/10 and has no witnesses. Suspect injury claim is invalid. Also missed workday before. Found no one to corraborate.

EE was putting up rib bolt; suction hose got damaged; took off glove reached down with his left hand to fix hose and a piece of metal poked thru top of left thumb.

The employee was helping to place tools in the electrician's tool box and the lid fell suddenly catching his finger between the lid and tool box frame. The incident resulted in an injury needing sutures.

Medical received 8/12/2009 the employee was setting a de-watering pump and strained his lower back. There was no injury/illness report filed with the company. He has missed 9 days of work and continues to be off.

The company received medical notification on 8/12/2009 that the employee alleged he was struck by an I-beam in the stomach. He missed four (4) days and has returned to work. Please note that an injury/illness report was not filed.

The employee was pulling and pushing on miner cable and strained his back. The event resulted in a lost time injury.

The employee stated that he was lifting and moving a 58-HP de-watering pump and strained his lower back.

The employee stated he was struck in the head by a falling timber. He was treated at the Boone ER where he was diagnosed with a strain/sprain.

This is an alleged event with inconsistencies related to reporting, dates and injury status. The employee was interviewed in the course of the investigation and between him and results of the investigation there is no known injury at this time that occurred while on the job during the specific dates given by the employee.

Employee was bent over moving a water line. When he lifted up, his back popped.

The employee was assisting inspecting the Joy 14CM15 miner MMU 005 #206 miner. When he pulled the hinge pin to drop the rub rail, the panel guard fell out of position and caught his right middle finger between the rub rail and guard. The incident resulted in a medical / lost time injury.

The EE was walking underneath belt cross under guard (belt not running) and making preparation to change out a roller. The belt slack was "let-out" at the take-up unit, causing the cross under guard and roller to contact the EE. The event resulted in a lost time injury. The EE received bruising of the right rib cage area.

The employee was lifting and handling rock dust (40 lb. bags) and began to experience swellling in the groin area.

The employee was removing a drill steel bit from inside the mine roof, when the drill steel he was using slipped out of his left hand and fell, striking him on his right hand lying on drill pot. The incident resulted in a cut/suture injury.

The employee stated he was supplying the roof bolt machine and when he lifted one metal strap onto the tray, he immediately felt a sharp pain in his lower back.
