The employee was attempting to pull a metal band out that was underneath a bundle of timber when he felt pain in his back. This was diagnosed as a back sprain.
Employee was bolting the previous round. He bent over to pick up a bolt and when he stood back up a small particle of rust or metal that had collected around the edge of his safety glasses fell into his eye and became embedded.
EE was in the process of clearing a blockage in the shaft pocket. He was standing on the pocket floor but was resting one foot on a reinforcement rod which was in close proximity to the shaft gate. EE closed the gate but did not realize that there was not enough clearance between his foot and the bolts that connect the gate to the ram. It caught his toe and caused a fracture
Employee was unloading cribbing from a timber skip in the raise and staging it beside him to move into the stope. This material is approximately 5' long and weighs 30 - 40 lbs. As he unloaded a piece he felt a pain in his back which was diagnosed as a sprain.
EE turned on the air valve for a device called a whiz bang which uses compressed air for ventilating and cooling. A particle of rust or metal blew into his eye and had to be scraped out.
EE was changing a locomotive draw bar to a different style. He leaned the first drawbar against the loci and while he was positioning the second bar the first one fell and struck his little finger. This accident was not reported until 9/5/12 and we just got a diagnosis 9/25/12 due to his having an anomalous physiology resulting from a birth defect on that hand.
Employee was working at the top shaft station. A crew of painters were doing elevated work out of a man basket adjacent to EE's position. The painters pulled some slack in a hose which caused it to dislodge a piece of angle iron which fell and hit a sloped roof, deflecting it and causing it to hit employee on the helmet. He incurred a neck strain and a laceration on his elbow.
The employee noticed on 6/16/12 that his eye was irritated and small metal fragments were found in his eye by a physician. He was not aware of how or when the material the got there but since he had worked the night before he assumed that it was from something he did at work. He had not filed an accident report.
The employee was attempting to remove a roller rack for a slusher from a raise skip when he lost his grip with one hand and the full weight of the equipment was suddenly transferred to the remaining hand. The roller rack weighs approximately 70 to 85 lbs.
The employee was attempting to move a rock that weighed approximately 40 lbs to use it for a fulcrum to extricate rock bolts that were buried in mud. He was not in a good position to lift the rock and it caused him to rotate his arm while lifting and this is what caused the injury.
Employee was attempting to pull the leg of a jackleg drill out of the muck when he strained his lower back. The leg had sunk into the muckpile while he was drilling the face.
Employee was rerailing a timber truck using the side stake on the truck for leverage. He did not have sufficient clearance from another timber truck so when he dropped it on the rail it pinched his hand between the stake and the other timber truck. This accident was originally reported by the employee as a first aid, but it was determined on 4/2/12 There was a treatable injury.
EE was guiding the rod eye of a hydraulic cylinder with his hand in order to place blocking under the cylinder body when the rod suddenly and unexpectedly extended pinching his thumb between the rod eye and the wood supporting the boom. This could have been avoided by following our policy of using pry bars for this activity and cycling the cylinder to flush out air locks.
Employee was using hot gloves to cover power lines with 2 halves of 6" x 10' plastic pipe. The top half slipped out of his hands and hit him on the side of the head.
Hoisting powder truck to put on the track to unload it. When hoisted up he pushed it over to the track and right shoulder popped when he went to unload it at powder mag and felt pain in right shoulder
Employee was placing a bag of tools into a pickup when his arm contacted a fractured tail light lens. A sliver of plastic became embedded in his right forearm which required excision and sutures.
Attaching drawbar for timber truck to locomotive, caught "R" middle finger between pin and frame of timber truck - lacerating tip of "R" middle finger.
Knocking a bit from end of steel, the steel fell over causing the end of the steel to flip up and hit EE in the mouth chipping two teeth.
Worker was unloading "holey boards" off of a timber truck. He had the hoist hooked to bands- when he went to move load-the bands broke, resulting in a cut finger & the load falling on his shin/foot.
Employee was helping explosive truck driver transfer the ANFO from truck to the explosives cars for shipping underground. After lifting a bag of ANFO employee felt "tug" in his back. Emp. is scheduled for four day week, was seen on Oct 28th and told to rest during 3-days off. Came back and saw Doctor on November 1st, was put on restricted duty.
Unloading supplies off timber truck on 4300 level, got something in "L" eye. Irritated all night long, went to ER next day and eye was rinsed out.
Employee finished drilling raise-up round and was cleaning up with a blowpipe, when he got a foreign object in his "R" eye.
EE was in 4600-169RS supporting a breast block to begin wedging it and felt pain to "R" shoulder. Breast plate weighed approximately 15 - 20 lbs.
#3 Top Station - EE was attempting to align an empty powder truck with the conveyance to load it, slid the truck around by grabbing and moving with his hands. Unaware the forks of the lift truck were close. EE caught "R" hand between powder truck and fork on forklift . Laceration (4 sutures) to back of "R" hand.
Timber skip hung up in raise, miner wiggled to move the skip down, there was 6 feet of slack cable above. Timber skip released coming down on EE "R" big toe fracturing the big toe.
Employee was bent over working on a 15 h.p. slusher and back popped.
EE was moving a diesl fuel truck into the 4600 fuel station, unaware that he was being helped by another employee. The other employee pushed on the fuel truck, catching EE's hand between fuel tank and rib. Laceration to top "R" hand.
Employee was working in the raise cleaning up. He reached out and grabbed a piece of lagging. When he lifted it up, he felt a pop and pain in his shoulder. He continued to work about 4 more hours before the pain became intolerable.
Employee was loading a 10" x 10" x 10' timber in a raise skip straining his lower back muscles on 03/08/2010. Employee continued to work his regular shift that day and for several days after. On 03/12/2010 employee asked to see a doctor for continued lower back pain.
Employee engaged in improper lifting, straining back and chest muscles.

Employee was unloading supplies from the cage on the 4900 level, when he strained his back from improper lifting.
Employee was opening the lid of a battery box on an electric locomotive. His grip slipped and he caught his hand between the lid and the side of the battery box resulting in a cut finger requiring sutures.
Employee was at the face unhooking a slusher block. A rock above the block fell striking he above the arm gaurd resulting in a laceration requiring sutures.
Employee grabbed the end of a hand held power hack saw lacerating finger.
Employee was standing in the mud of a sump pulling on a pump hose. It broke free and employee could not move his feet causing him to wrench his back.
Employee stumbled on a rock and hit shin on slusher bucket causing a large hematoma.
Employee doing track repair, bent down and couldn't get up. As of January 27, 2009 this case was denied by the State of Idaho that the employees condition was not the result of an industrial injury but a continuation of a pre-existing condition.
Employee was helping lower concrete forms for the next shaft pour. The form became hung up and then sprung free crushing the employees hand between the steel form and the concrete shaft.
Employee was unloading rockbolts when the metal band on the bundle broke striking the employee in the Rt. Forearm causing a deep laceration.

EE was loading bolting mats into cage. The EE lost control of the mats he was loading, catching his hand between the mats and cage door. Injury was a cut on index finger, requiring 2 stitches.
