Showing all 22EE was putting the plates on the cable roof bolts when some grout came off the cable and fell in eye. EE has a lacerated cornea and received antibiotic drops.
EE was hanging pipe back up when the chain slipped off the pipe. The pipe struck the EE and bent him backwards causing pain in his stomach and shoulder. EE has a hernia and a strained shoulder.
EE mixed "The Works" toilet bowl cleaner, blean and GoJo handcleaner together to mop the floor of the lamp room. EE experience breathing problems as a result and was transported to hospital for treatment.
EE was trying to remove grizzly bar from stacker belt. Turned on magnet to help lift from belt. Two middle fingers got caught between magnet and bar. EE was wearing gloves but tip of middle finger was amputated and finger was broken.
EE stopped to roll a rock out of the way of drilling a lifter hole. While rolling the rock he felt a pain in his back. Pain worsened over next 24 hours. He is now on restricted duty.
EE was helping bolt a hydraulic hose manifold on a boom. While EE has handing manifold up, the other EE slipped off drill and landed on first EE. EE recieved a shoulder strain and lumbar strain. EE received restrictions in which he could not do his job fully on 4/8/08.
EE was replacing belt clips on discharge conveyor, when dried mud fell onto his face. When EE tried to wipe the material off of his face, a small piece fell behind his safety glasses and into his eye. Debris was removed and an antibiotic ointment given.
EE was helping to lift 120 pound grease barrel into the back of a pickup, when he had lower back pain.(EE went back on restrictions on 5/14/08.)
During reassembly of new equipment, EE hurt his knee.
Employee was helping to unload a parking brake (for a loader) and place it on a two wheel dolly. The brake assembly slipped and caught the tip of his left middle finger. The doctor bandaged the finger and prescribed medication.
Cleaning tail gate of dump truck, his right thumb caught in tail gate. Reportable fracture, right thumb; lost time.
Dumping bucket, bucket swung to side mashing right hand between bucket and car causing dislocation of right thumb, possible torn cartilage.

Employee had beam lying on saw table rollers. He lifted the end to align it with the saw and strained his back. Doctor placed man on temporary restricted duty, not to lift over10 lbs., and was given medications.
Employee was unloading a powder truck. Apparently, he was twisting while picking up boxes of powder. When employee complained of back pain, we took him to the doctor for evaluation. Doctors evaluation was Lumbar Strain. Placed on temporary restriction to not lift over 10 lbs. given Presctiption Medication.
Employee was working in the lab. He broke a burett containing Nitric Acid. The acid splashed on his chest resulting in minor burns. Employee was treated at a medical facility and released.
Employee mashed middle finger on left hand repairing steering pump. Reportable due to stitches. No restrictions or lost time.
Employee was struck on right forearm by pipe section that was being installed. Stitches. Ret'd to work-no restricted or lost workdays.
