EE was hanging CM cable and sharp pain felt in right bicep.Scans revealed tear in tendon/muscle in right bicep.EE is going to physical therapy and surgery may be required.This report is filed late because individual whom normally filed 7000-1 reports was recently terminated and the person filing this report discovered on 7/15/2013 that it had not been reported.

Employee was helping loading rail on flatcar when left middle finger got caught between two rails pinching finger. Was seen at ER 8 sutures and positive fracture in three places on left middle finger.

EE was shoveling what he called heavy mud under beltline, twisted back by a turning motion to unload shovel on belt, he stated back went numb, was seen at ER for back strain.
Employee was Filling Oil Truck With Oil. He Was Pulling On Oil Hose And Pulled A Muscle In Right Shoulder. Employee Went To Family Urgent Care On 2-10-2012 And Receved Treatment.
Employee was beating bearing off of roller assembly on a fletcher RB,A piece of metal flew off striking right hand and becoming embedded under knuckle of index finger, He was seen by a doctor and DR. advised to leave metal in, that it posed no complications.
Employee went to Dr. over sore small finger, physician lanced and drained finger, found a small metal sliver embedded in finger, employee was unaware of injury and stated that he has not been working on anything metal, other then work, no sutures were placed, no lost time.

Employee attempted to lift a bulky portable rock duster into scoop bucket by himself. The result was a Hernia in abdomen/stomach area. Employee had surgery to repair the hernia.

Employee was in the process of removing a crib block, used as blocking material, from under the bumper of a continuous mining machine. The crib blocks placed under the stab jack rolled allowing the CM to fall catching the employee's right little finger between the bumper and crib block.
EE was building a stopping and while lifting blocks pulled muscle in left arm. 18 days restricted duty was found as a result of a Part 50 Audit.

Employee stated that a piece of drill steel was stuck in the drill hole, while attempting to remove the steel it dropped from the roof striking left hand, employee was seen by physician for contusions and abrasions, report is 355 days late as a result of a part 50 audit.

Employee was installing a 10" roof strap; his hand slipped and cut his left middle finger.
While working on a battery powered coal hauler, employee stated that his fingers became pinched between bolt and crib block.
