Employee was changing pump at sump when water entered his boots. Water entering sump contained portland cement from nearby pour. Employee received alkali burns to feet.

Miner was blow piping one of his lifter holes clean when he was struck in the mouth by a rock ejected from the hole, this resulted in a chip to the right front tooth.

Employee was moving some large rocks into a pile on a bank. He set his hand down and one of the rocks rolled off the pile onto the rock his hand was set on, cutting the inside of his left pinky finger to the bone.

When the employee reached the second landing in the manway a 3' x 12' piece of timber fell over and struck her on the hard hat. The employee was released to full duty at the time of incident. The employee was still sore and went back to the doctor, at that time she was placed on modified duty.

EE had just finished drilling a round when he set his jack-leg drill up against the rib so he could start cleaning his lifter holes. The machine was not secure when it tipped over and landed on ee right hand causing a laceration between the ring and little finger that required stitches.

On March 1st, 2011, EE was connecting pipe to the concrete pump at the 5050 East Lateral Draw Point #2. EE was connecting a steel pipe to a rubber concrete hose. While holding the lines in place and attempting to close the clamp around the connection, EE felt a pain in his back.

While installing a 4' Split set bolt, the miner looked up and felt something contact his left eye. Miner flushed out eye, still felt something in eye. Miner was taken to doctor. Eye flushed out and given eye drops. Miner was also given an antibiotic ointment, which constitutes this report.

Employee reported troubled breathing at the start of shift, but said would be OK. Employee was sweeping the floor in the building and was still having trouble breathing and having some chest discomfort. Employee was then taken to doctor in town. Potential exposure to cleaning chemicals may have triggered the troubled breathing. Released light duty, constituting this report.

This employee was involved in an argument with another employee. This employee attempted to open the door on a haul truck that the other employee was operating. When he did the truck driver kicked open the door while this employee still had a hold of the door handle. The result was a broken left ring finger.
