Employee stated he bent over and moved the continuous miner cable slack out of the roadway, and when he stood up he felt pain in his back. Employee was diagnosed with a back strain and taken off work. The operator of the coal mine at the time of the incident was Gateway Eagle Coal Company, LLC.

The employee stated that he was carrying labor saver cinder block when the block suddenly broke; shift the weight to his left hand, which caused pain in his lower back. He continued to work. However the pain did not subside, which prompted him to seek medical attention. He was diagnosed with a back strain and placed on modified duty for 3 days.

Employee stated that as he was attempting to close the battery lid on a scoop, he lost his grip on the lid and the lid struck his left hand resulting in a fracture.

On 9/10/15, while the mine was owned and operated by Emerald Processing, LLC, the individual stated while he removing drill steel from a previously drilled hole he experienced pain in his right shoulder. The incident did not become reportable until 11/7/15 when the individual initially sought medical attention, at which time the mine was owned and operated by Kanawha Eagle Mining.

Employee stated that while removing a top roller and structure from the longwall belt he felt pain in his left elbow. Employee continued to work and did not begin to miss work until 9/18/2015 when he was diagnosed with a sprain/strain type injury to his elbow.

The injured was plastering a stopping when some liquid off of the plaster splashed in between his safety glasses and his face getting into his left eye. The injured was prescribed prescription eye drops.

Employee stated that he was loading a 5 gallon hydraulic oil can on a pallet and he twisted his back. He said he felt a pop in his lower back. Employee went to the doctor over the weekend and was diagnosed with a pulled muscle. He notified the mine site on 9-1-15 that he would be on light duty for 1 week. Employee returned to regular duty on 9-8-15

EE stated that he was reattaching a wire to the starter of the 910 mantrip at the time of the incident. He stated that he had to reach under the shields during the process and strained his shoulder and the pain intensified after servicing the wheel unit of a shuttle car. EE later sought medical attention and diagnosed with a strain. He was placed on restricted duty.

Employee states he went to the Brody bathhouse after his shift to clean-up. He was attempting to turn the water supply to the sink on when the incident occurred. He placed his left knee on the sink and reached up to turn the valve. The sink fell off of the wall and broke, resulting in a laceration to his left shin. Sutures were used to close the wound.

The employee stated that he was adding coolant to the 507 diesel scoop due to the scoop overheating. When he was adding the coolant to the scoop, the coolant boiled out, contacting his right arm. On 8/17/2015 the employee sought medical attention and was prescribed antibiotics for an infection.

Employee stated that he was attempting to hang a bulk bag of rockdust while standing beside the scoop bucket. He was stretched out attempting to hook chains through the bag when he felt a pain in his uppper back between his shoulder blades.

The employee stated that he got out of his scoop to load 6 crib blocks and when he lifted and turned with the last block he felt pain in his lower back. The employee finished his shift, but later sought medical attention at which time he was diagnosed with a strain. He returned on his next shift under restricted duty.

EE stated that he was in #5 entry hanging shuttle car anchor when right arm hyperextended backward. Been attempting to place a pin in a shackle with left hand while trying to hold anchor with right. Incident became reportable 8/30/2015 follow-up diagnosed the ee with a torn tendon. EE returned to work at his normal job class. On 10/14/15 ee began missing work due to the injury

Employee was helping change battery cells out of the #523 scoop on the 18 headgate longwall section. In the process of loading one of the battery cells into the #765 Duckbill Mantrip, the employee felt pain in his right arm.

Employee states he was turning the ammonia off on the B-side thickener and the valve leaked and sprayed ammonia on him. Employee went to Boone Memorial Hospital and referred to a Specialist. Employee was released to return to work. On 7/30/2015 the employee missed work due to an adverse reaction with the antibiotic that was prescribed.

Employee was helping install belt structure when his leg gave way and employee started to experience pain. As he was driving home, he experienced pain again when pressing the brake pedal. A subsequent MRI showed no acute findings for the lower back area but noted degenerative changes. Note: This employee has a record of several occasions with experiencing back pain

The employee stated that he and another employee were in the process of loading a scoop with rock dust and during the process the employee got rock dust into his eye. The employee later sought medical attention and was prescribed prescription eye drops.

Employee states he was mounting #21133 930H Loader and felt a pop in his right knee. Employee was treated at Boone Memorial Hospital on 7/14/2015 and 7/22/15 and given an MRI. On 8/17/2015 the employee was treated at Scott Orthopedic by Dr. and diagnosed with a torn meniscus and taken off work.

Employee was removing a worn out piece of metal liner from the raw coal feeder. The employee removed the metal from the feeder and went to throw the piece of metal out of the way. When the employee threw the metal it cut his left index finger.

Employee stated that he was cleaning his permissible ride and attempting to water the batteries. The ride has a spare tire attached to the battery lid cover and when he moved the spare tire, he felt a muscle strain in his back. The employee finished his shift but he went to the doctor on 3-15-15. He was placed on light duty for 1 week with a return to full duty on 3-23-15.

The employee stated that he had just completed dumping the metal from the slope belt magnet and as he was raising the metal door back into position, he felt pain in his left shoulder. The incident became reportable on 3/16/2015 when the employee did not return to work.
