Employee was lifting a box of resin cartridges onto a shelf about 40 inches from the sill felt a sharp pain in back. Was taken to hospital and placed on modified duty until the results of an MRI are reviewed.

Showing all 4Employee was cleaning out a diesel fuel line with brake cleaner. While spraying the fuel line with the brake cleaner the cleaner ricocheted back into the employees face. Brake cleaner also went up under the employees safety glasses and into both of employees eyes. This caused a burning sensation, pain and loss of vision to both eyes
Employee was holding a 6" tailings decant water line that had burst at a splice, and was trying to direct the water towards a small water impoundment. The water line was to heavy for the employee and he dropped the hose. When he did so the banding from the splice cut his right hand ring finger.
Employees left foot was caught in hoses, employee jerked up with his left leg to free it from the hoses and struck his knee against a ladder. Injuring his left knee. It was latter found out that employee had surgery on his knee, but had not informed anyone at the mine site. Employee has been released to modified duty but has chosen not to return to work.
