While pulling down exhaust curtain with their left hand in Unit 1, Entry 12, 44+40 - 45+00. In doing so, a section of rock fell out, striking the contractor on their head and left side.
While lifting a belt crossover to back on it's legs it slipped. This caused the employee to jam their arm/elbow into the crossover legs/skirt boards on the pony drive in Unit 3.
Employee was unloading a box of SCSR's off the back of a Kubota when it slipped. Employee heard pop in left arm around elbow area and felt pain/numbness. Employee continued to work until surgery on 10/31/2025.
While carrying belt structure, employee felt pain in right arm. They tripped while carrying a top roller and felt pop in right elbow/forearm area.

Employee felt pain in EE's side and abdomen and was evaluated in August 2022. It was diagnosed as a non-work related strain. EE continued to work until EE was re-evaluated. EE was taken off for medical treatment in May 2023.
Employee had removed the suction plate on the bottom of the roofbolter drill pot and was in the process of re-installing this plate. While installing the plate it slipped out of their grip, falling approximately 16 inches contacting their shoulder. Employee continued to work until until 08/10/2022 when they were placed on restrictive duty by a doctor due to discomfort in shoulder.
Employee was picking up a belt roller cradle when they felt a pop in their shoulder. This employee continued to worked until 08/29/2022 when they were taken off work by a medical doctor.
While performing service work on a continuous miner, the EE raised a hinged door and rested it on their shoulder. The EE had slight body movement, causing the door to slip and fall onto their left hand. This resulted in a fracture of the left hand.
Contractor employee was building a stopping using concrete blocks. Contractor picked up a concrete block and put it in place when they felt pain in shoulder
Contractor employee was helping load angle iron metal pieces when one the pieces rolled and contacted EE's right pinky finger resulting in a fracture.
Employee was hand loading a valve bank for a U/G roof bolter. When they loaded the valve bank on to the transportation vehicle it shifted causing the employee to strain their back. The employee continued to work until they were taken off work on 03/10/2022 for medical treatment.
IW was working next to another EE when the other EE was unhooking chains from material. The chains stored their own energy and as such when the last chain was unhooked from position it swung toward the IW striking the IW in the mouth causing laceration with repair by sutures.
Employee experienced some pain in their back and right arm as EE was helping hang miner cable. This incident was reported on 09/22/2021, employee continued to work until 10/12/2021 when EE was taken off work by their doctor for treatment.
Employee was assisting with removing the jack pipes from the unit tail piece during a belt advancement. As employee pulled the pipe out of the rib notch, employee felt pain in groin. After medical evaluation it was determined that employee has a hernia that does not require medical treatment at present. This incident occurred on 8/18/2021.
Employee was performing maintenance duties. EE was moving a wheel hub and felt a pop in EE's groin area on 03/20/2021. Due to medical opinion EE was removed from work on 04/02/2021.
In the process of pulling a rock off of the Continuous Miner a piece of the rock came off and contacted the employee; The result of this was a laceration over the left eye requiring stitches.
Employee was doing gunite work on the East Chevron doors, EE was climbing up a ladder pulling the hose with them. EE's head contacted the mine roof jamming head and neck. This incident was reported on 12/10/2020, employee continued to work until they were taken off work by a doctor on 06/28/2021.
While putting bolts in a top roller. The bracket came down when they pulled it into them fracturing the left index finger and smashing the right thumb.
