EE was lifting a lid to a bag house. EE was lifting with one hand while grabbing on to a hand rail with the other hand and as they sat the lid back down is when they started complaining of back pain. Employee did complete the rest of shift that night and went on days off. On Friday they reported that they needed to see a doctor and was taken to the clinic.

Employee was repairing a loadout spout. While moving the spout the employee's finger got pinched between work platform and the bottom of the spout causing a cut on left hand ring finger that required 3 stitches.

The injured as removing a counter weight from a excavator after it was on the ground it appears the ground gave way because of ice and there was slack in the chains, as the weight tipped and chains tightened it broke a couple bolts and cylinder bracket and fell to ground

While doing maintenance on the dryer a chain fall broke loose and struck a worker in the safety glasses. The impact caused safety glasses to cut cheek and nose requiring stitches

Employee was fueling equipment when the diesel fuel hose failed. The hose developed a hole causing diesel fuel to come in contact with the employee's eyes. The employee was wearing safety glasses at the time of the incident.

Two employees were digging out sand beneath a conveyor that carries fine sand from the Hydrosizer to the sand pile. The employees were attempting to locate a power line to repair a short. One operator struck a metal support with shovel head when ee attempted to dig into the dirt. The impact caused a sprain/strain in left wrist.

Employee was placing a counterweight object on a water hose to prevent the hose from "wiping". EE's gloves were wet and EE dropped the object on EE's right hand.

While diver was outside the truck cleaning the mud flaps, EE noticed a turnbuckle stuck in the tail gate. EE tried to free the turnbuckle by lifting the tail gate with left hand. When the turnbuckle came free the gate shut smashing 4 fingers on the left hand.

On Tuesday 11/19/13 EE reported to his lead, that he was experiencing severe pain in his abdomen area. He said that on Friday he and a co-worker were changing the upper Rotex screens and he felt some pain in his stomach but thought nothing of it. EE had surgery to repair a hernia. He will be off at least ten days and will be able to return to work with restrictions.

Employee was moving one gin pole from one sheet to another when he felt it slip. Instead of dropping it, he grabbed tighter and pulled a muscle.

Employee was attempting to move one end of a tank panel by himself. The vertical bolts got stuck on the sheet beneath the one he was attempting to move. As he jerked on the sheet he felt a pull in his back. He reported to me a few minutes later that he could feel his back hurting. I then took him to the clinic and the doctor revealed a pulled muscle.
