Employee was throwing low belt structure and pinched EE's right pinky finger in between two belt rails, breaking their finger.
Employee was pulling low structure stand to straighten the belt line and felt a pop in their back. Employee was diagnosed with several bulged discs and placed on restricted duty.
Employee was advancing canvas. While walking employee's left arm contacted a roof bolt plate causing a laceration resulting in four stitches.
The employee was struck in the bottom lip by a drill steel while attempting to remove steels from the roof. They received a laceration that required three stitches.
The employee was stretching out carlon pipe. When trying to secure the end of the pipe it slipped out of employee's hand and struck the employee in the upper lip. The employee received four stitches to close the laceration.
Employee was assisting with a cable repair. Another EE dropped a knife in a pile of rockdust. Injured employee reached into the pile of dust to get the knife a received a cut on the RT middle finger that required 4 stitches. The area is low and wet.
Traveling to section seen a bucket of miner bits in travel way stopped to pick them up from cart and felt left shoulder pop while lifting bucket of bits resulting in a rotator cuff tear. Continued working full duty until time of surgery on 07/08/2024.
Employee was loading T-3 channels onto a scoop. The stack of T3's started to tip over so they tried to catch them. Their hand went into the bolt hole and was cut by the straps as they shifted causing a laceration to their right pinky finger that required stitches. The employee was wearing rubber gloves when the accident occurred.
Employee was bending a 6' roof bolt when they felt a sharp pain in left shoulder. The employee has continued to work regular duties and treat conservatively until 1/10/24, at which time it was determined surgery would be required to repair a partially torn labrum. Surgery scheduled for 2/6/24.
Employee was shoveling a belt tail. As EE pushed EE's shovel, it stopped suddenly and the employee hit EE's hand on something causing pain to EE's right hand. The employee did not request medical treatment until 11/16/23 at which time EE was sent for an x-ray and diagnosed with a fracture.
Employee was attempting to use bed rail for leverage to put dolly back on track. EE picked rail up to put under dolly and felt a "pop" in lower, middle part of back. EE was not exerting any force at the time of injury. Conditions were low and dry.
Employee was loading T3 channels onto a skid when they felt a strain in their abdomen. The employee continued to work full duty until they were seen by a medical professional who diagnosed them with a hernia. Employee was given restrictions on 10/17/23.
Employee was carrying an oil can and pump can back to EE's ride when EE felt a pinch in EE's back.
Miner stuck finger between the wheel and the hub and got it pinched.
Employee was removing valve bank from MBC. While holding valve bank ee felt pop in RT bicep. Employee felt a tingling sensation and pain when trying to straighten arm.
The employee was making the second bend on a six foot bolt when EE felt a pop in the right shoulder.
An employee threw an unsupported roof sign to the miner operator and accidently struck them above the right eyebrow. This resulted in a laceration requiring 5 stitches.
The employee overextended EE's lower back while bending a six foot resin bolt.
Employee was holding the offside rub rail on miner while rub rail was being installed. Rub rail sprung back jarring EE shoulder. EE continued to work full duty until date of surgery while diagnostic procedures were being completed and diagnosis was confirmed. Surgery completed 12/14/2023.
Contractor was lowering spacer blocks behind a bearing on the 6-3 nose roller. The spacer block slipped and contractor's finger slipped into 1 of the bolt holes on the spacer block causing a laceration to the left ring finger requiring sutures.
THE BOLTER OPERATOR HAD BENT A BOLT DURING INSTALLATION. THEY CUT ABOUT HALFWAY THROUGH THE BOLT AND BEGAN PUSHING IT TO BREAK IT OFF. THE EMPLOYEE GRABBED THE BOLT FROM THE OTHER SIDE AND BEGAN PULLING TO ASSIST THE OPERATOR. THE BOLT BROKE CAUSING THE EE TO FALL BACKWARDS AND THE PLATE ON THE BOLT STRUCK THEIR ARM BETWEEN THE WRIST AND FOREARM CAUSING A LACERATION.
The employee was trying to spin a roll of belt by self. After doing so, the employee stated something didn't feel right, a little while later EE noticed a lump in groin area.
EE was heating up tar for scoop batteries using a tea pot and hot plate. EE opened the tea pot to check how much tar was in it. When EE lifted the lid it blew out of the container covering the palm of EE's hand and between fingers migrating to the wrist area.
Employee was roof bolting and felt a pain in EE's back. EE was given a cortisone injection in EE's back to help alleviate the inflammation.
EMPLOYEE WAS USING B-BOND CEMENT TO SEAL KNEE WALLS. EE RECEIVED IRRITATION/BURN ON BOTH FOREARMS AND KNEES FROM CONTACT WITH THE CEMENT DESPITE THE USE OF PROPER PPE AND WASHING. EE HAD BEEN TRAINED ON THE USE AND HAZARDS OF B BOND.
The employee was bending a six foot torque tension bolt. While applying downward pressure on the bolt, it slipped out of EE's hands and sprung upward striking EE in the bottom lip causing a laceration.
Employee was advancing 6" waterline to the section. While lifting a 6" metal sweep adjacent to the section belt drive, the employee felt a sharp pain in lower back.
Employee was helping with a belt advancement. As they were throwing a bottom stand under the belt, they felt a "pop" in left bicep. Employee was diagnosed with a partially torn bicep and continued to work regular duties until surgery date on 2/9/2022.
EE was bending a 6' bolt. When EE was pushing on the bolt, the bolt shifted and the employee fell forward and strained right shoulder. EE was working regular duties until seeing doctor on 12/28/21 and being placed on light duty. Surgery scheduled for February 2022.
Employee was moving the forks on a skid loader. As EE was sliding the fork, EE pinched EE's finger between the fork and the carriage resulting in a small fracture to EE's left thumb.
EMPLOYEE WAS ADDING OIL TO THE BOLTER. EE HAD OPENED THE OIL CAN AND PLACED THE HOSE IN . WHEN THE CAN BEGAN TO RUN EMPTY EE REACHED OVER TO TILT THE CAN AND EE'S LEFT THUMB CONTACTED THE SHARP EDGE OF THE LID. EE RECEIVED 6 SUTURES FOR A LACERATION
Injured miner was crossing over top of the haulage unit. When EE grabbed the the metal structure to pull self up onto the structure, EE dislocated right shoulder.
EMPLOYEE WAS SHOVELING LOW BELT. EE FELT DISCOMFORT IN BACK. EE STRETCHCHED BACK OUT AND AFTERWARD HAD NO STRENGTH IN LEFT ARM/SHOULDER. EE CONTINUED TO WORK FULL DUTY UNTIL 8/18/2021 WHEN EE WAS PLACED OFF WORK.
Employee was cleaning rock off of the miner. When EE was pushing rock off edge of the miner it pivoted up and caught EE's right middle finger between the rock and the roof causing a laceration to the tip of the finger. The employee received 5 stitches.
Employee was throwing out belt structure when EE felt a pain in right Bicep. Employee worked normal duty until 10-13-20 when EE seen a doctor and was placed on restricted duty work. Employee has a ruptured bicep.
Employee was removing a rock from the MBC coal chain when the rock slipped it pinched employee's thumb between the rock and coal chain. Employee received 9 stitches to close the laceration.
EMPLOYEE WAS ROOF BOLTING AND BENDING A BOLT FOR INSTALLATION. EMPLOYEE NEVER FELT ANY TYPE OF INDICATION BUT WHEN EMPLOYEE GRABBED THE STEELS FOR THE NEXT HOLE A BURNING PAIN WAS SHOOTING UP EMPLOYEE'S ARM FROM WRIST TOWARD ELBOW. SWELLING BEGAN IMMEDIATELY IN THE ELBOW AREA.
Injured miner lifted a resin box and placed it on the roof bolter. As EE lifted the box, EE felt a sharp pain in EE's lower back and the pain radiated up EE's spine toward EE's neck.
Crib tower was being built. Employees were throwing cribs in a fire brigade manner. Injured employee was holding one crib when 2nd crib was thrown to employee. This smashed employee's right pinky finger and caused a crushing injury that made an open wound that required 4 stitches.
Employee was lifting a rail onto stinger when EE felt a pull in EE's left groin area. employee received a hernia. surgery is 3-10-20.
EMPLOYEE WAS APPLYING WET CEMENT TO A WALL. CEMENT GOT INSIDE OF EMPLOYEE'S MUD GLOVES CAUSING A CHEMICAL BURN/IRRITATION.
EMPLOYEE WAS MOVING A BRAIDED STEEL WATERLINE ON THE HAULAGE UNIT. A STRAND OF THE METAL BRAIDING WAS PROTRUDING FROM THE HOSE . WHEN THE EMPLOYEE GRABBED THE HOSE THE STRAND OF WIRE PUNCTURED THE EMPLOYEE'S RIGHT LITTLE FINGER. EMPLOYEE WAS TAKEN TO HOSPITAL ON 7/26 AND GIVEN INTRAVENOUS ANTIBIOTICS DUE TO FINGER BECOMING RED AND SWOLLEN.
Employee was dragging a section of bridge cable out of the way when employee felt a sharp pain in stomach/groin area. Employee has a hernia.
Employee grabbed ahold of handle on drill station canopy and went to stand up when shoulder dislocated. Employee received medical treatment to have shoulder set back in place.
Contract employee was picking up motors at PBS Warehouse. The motor was frozen to the ground and when Ee attempted to lift it and hit the pallet to break it free, the hook came undone and struck employee above left eye causing laceration.
Employee was getting on mantrip when ee grabbed a hold of building there was a sharp piece of tin sticking out that caused a laceration to left hand. Employee received 6 stitches to close the laceration.
Employee was in the process of emptying a trash can in the shop. EE strained stomach as EE attempted to lift the trash can.
The employee was plastering a concrete brattice. While moving the bucket of plaster, the employee dropped the bucket and the plaster splashed into the employee's right eye. The employee worked the remainder of shift. On 11/20/2018 the employee's right eye was red and slightly swelled. The employee received medical treatment for the eye injury on 11/20/2018.
Employee was straightening a 6' roof bolt to complete the installation. The bolt spun and employee twisted back.
Lifting low belt structure stand and swinging it with both arms to place it underneath the conveyor belt, the stand hit a ledge or mud and suddenly stopped movement. This caused a severe "pop" sound from employee's right inside elbow and lower bicep area.
