Employee was loading T-3 channels onto a scoop. The stack of T3's started to tip over so they tried to catch them. Their hand went into the bolt hole and was cut by the straps as they shifted causing a laceration to their right pinky finger that required stitches. The employee was wearing rubber gloves when the accident occurred.

Employee was bending a 6' roof bolt when they felt a sharp pain in left shoulder. The employee has continued to work regular duties and treat conservatively until 1/10/24, at which time it was determined surgery would be required to repair a partially torn labrum. Surgery scheduled for 2/6/24.

Employee was shoveling a belt tail. As EE pushed EE's shovel, it stopped suddenly and the employee hit EE's hand on something causing pain to EE's right hand. The employee did not request medical treatment until 11/16/23 at which time EE was sent for an x-ray and diagnosed with a fracture.

Contractor was lowering spacer blocks behind a bearing on the 6-3 nose roller. The spacer block slipped and contractor's finger slipped into 1 of the bolt holes on the spacer block causing a laceration to the left ring finger requiring sutures.

THE BOLTER OPERATOR HAD BENT A BOLT DURING INSTALLATION. THEY CUT ABOUT HALFWAY THROUGH THE BOLT AND BEGAN PUSHING IT TO BREAK IT OFF. THE EMPLOYEE GRABBED THE BOLT FROM THE OTHER SIDE AND BEGAN PULLING TO ASSIST THE OPERATOR. THE BOLT BROKE CAUSING THE EE TO FALL BACKWARDS AND THE PLATE ON THE BOLT STRUCK THEIR ARM BETWEEN THE WRIST AND FOREARM CAUSING A LACERATION.

EE was heating up tar for scoop batteries using a tea pot and hot plate. EE opened the tea pot to check how much tar was in it. When EE lifted the lid it blew out of the container covering the palm of EE's hand and between fingers migrating to the wrist area.

WHEN THE EMPLOYEE GRABBED THE HOSE THE STRAND OF WIRE PUNCTURED THE EMPLOYEE'S RIGHT LITTLE FINGER. EMPLOYEE WAS TAKEN TO HOSPITAL ON 7/26 AND GIVEN INTRAVENOUS ANTIBIOTICS DUE TO FINGER BECOMING RED AND SWOLLEN.

The employee worked the remainder of shift. On 11/20/2018 the employee's right eye was red and slightly swelled. The employee received medical treatment for the eye injury on 11/20/2018.

Lifting low belt structure stand and swinging it with both arms to place it underneath the conveyor belt, the stand hit a ledge or mud and suddenly stopped movement. This caused a severe "pop" sound from employee's right inside elbow and lower bicep area.
