A temporary staffing service employee was washing a 250ml Erlenmeyer flask with a bottle brush. During this process the flask broke and a piece of glass penetrated a nitrile glove and cut the palm of the injured employee's right hand. The injury resulted in the employee receiving 6ea. stitches and a return to work release with no work restrictions.

100 lb. barite sack rolled onto shoulder of injured temporary staffing employee. Diagnosis of other sprain of left shoulder joint - minor and Pain in left shoulder - minor. Doctor released with no restrictions on 12/11/15. Sore shoulder prompts doctor's visit on 12/17/15 resulting in "Medium Work" restriction and left shoulder/scapular contusion diagnosis.

Employee was cleaning the plant area, rolled a 2" x ~15' suction hose, and placed on the floor. At the time the hose was placed on the floor he apparently twisted his back. An 11/9/15 physician's report indicates that the employee's diagnosis was "Lumbosacral strain / R Sciatic neuralgia", and states that the employee "is totally incapacitated at this time".

Worker was working by air compressor fixing jig screens. The extra jig screens were resting against compressor. The compressor fell over, breaking one of the fittings. It caused a blast of air, the worker ended up on the floor-five feet from original position, and a cloud of dust occurred. the worker got metal fragments in his eye. He had his safety glasses on.

Employee was shoveling mud (barite) up off the floor into the shaker (dewatering screen), which is about 5 foot high and his knee gave out on him. This is a repetition job to keep his area clear of hazards and clean as needed.
