Emplooyee claims cut left index finger in Production Shaft on 09/25/2020. Did not report until 10/02/2020 after finger became infected and required treatment. Finger was lanced and antibiotics prescribed. Employee placed on light duty restrictions.

Employee was operating a bag machine when EE pulled a bag back toward self to realign the bag with the sealer. EE states EE felt a "twinge" in EE's left shoulder when EE pulled the bag backwards.

Employee was lifting a 12" vinyl hose about 1' to clear the hose of a salt lump. When the employee pushed up, employee felt a pull sensation in shoulder.

Pulling on conveyor belt and strained lower back during belt replacement. Employee noticed stiffness in lower back next morning. and reported to supervisor Monday morning when they returned to work.

Employee received a laceration while picking up trash from the hoist yard by 3 shaft collar. The material picked up were remnants of brackets that were just removed from the shaft that had jagged edges.

Employee was picking up timbers and tossing them into the backhoe. Upon twisting to toss employee felt a strain in neck. Employee saw doctor and returned to full duty. Employee later began to experience problems and sought medical treatment resulting in surgery in January 2019. Incident became reportable on January 25, 2019 due to surgery.

Employee was driving Polaris into shop at the end of shift after performing work assignment of pinning down a feeder breaker from movement using H beam with holes to use to install cold rolls (pins) when they felt pain in the lower middle back.

11/14/16 the employee stated that ee was lifting a cover on an underground scoop when ee felt a "tweek" in back. Employee did not convey or realize the severity of injury and continued to work the next few days. 11/21/16 the employee filed an injury report and went to a doctor for an evaluation of back. The employee returned to regular duties with restrictions.

Employee and helper were installing the left side steering cylinder back pin into the cylinder from under the truck frame. While aligning the holes in the cylinder, frame and bearing, the cylinder pin rolled off the inner frame and fell to the ground striking the employee.

Allergic reaction to epoxy grout. While pouring epoxy grout for column foundation employee noticed skin irritation. EE had rubbed and scratched during the day, went to the Dr. after shift. Dr. treated for rash and infection.

Employee was moving a bag of ingredient off of a pallet and hit a fire extinguisher, knocking it off the wall onto the top of foot. Employee was examined at the ER, no broken bones just deep contusion. I overlooked employee's restrictions, therefore did not report this properly.

Employee alleges he incurred an umbilical hernia while coiling up fan cable. This cable weighs approximately 12.6 oz per foot. While in the act of coiling up cable the employee was bent over at the waist, the employee stated that he felt cramping in his abdomen and stood up and felt a burning sensation and a slight bulge in his abdomen.

Employee allegedly incurred a right shoulder strain while installing a fork lift seat. The employee was attempting to flip the seat over the parking brake and he stated the seat slipped from his grasp and he incurred a muscle strain to his right shoulder.

Employee allegedly incurred tennis elbow while moving two pieces of steel. (1 angle 3X3 X 1/4 in thick x 8 ft and 1 flat bar 4 in wide x 3/4 in x 8ft. The employee claims that he hurt himself 10 minutes into job. The employee was assigned to stack steel onto a metal rack.

Employee incurred a broken nose after being struck by a 2" waterline. The employee was moving the waterline and other utilities to prepare the back to be mechanically scaled. While the employee was in the process of strapping the waterline to a roof bolt plate the waterline popped out of the cable handler (U Shaped bracket in a high lift) and struck the employee in the face.

Employee states he was inspecting the centrifuge chute after cleaning with the inspection door closed. Employee removed the inspection door pin and opened the door. While holding the inspection pin in his right hand, the pin contacted an appendage on the rotating drum which slammed the metal door pin into the thumb of his right hand resulting in an injury.

Employee was attempting to stabilize the spare Derrick motor that was resting on a standard pallet, but was not resting solidly (cocked). The motor slipped off of the pallet pinching his finger between it and the floor.

Employee felt pain in back while removing 25lb tool bag from shelf. As employee was removing the bag, the bag slipped from his grasp and he attempted to grab the bag and experienced the pain in his back. Employee has experienced several incidents with chronic back pain.

A 3-1/2" x 4' steel shaft was being passed to employee by another employee. The shaft was not properly balanced, one end dropped approximately 6" jamming employee's right index finger between the shaft and the drive motor base of the elevator.

The Dissolver Circulation pump at Power & Production was locking up intermittently. (2) employees were sent to trouble shoot the pump. It was determined the pump would need to be opened. The pump was locked/tagged, isolated & bled. During the removal process when the pump was pried loose from the housing, hot brine was released under pressure that resulted in burns to both EEs.

EE was working on bolter at Morton Salt. he says as he was positing the work basket I mistakenly operate the lever to close the lower centralizer. My left index finger was caught in the jaws. Crushing my left index finger, resulting in broken bones (index finger) and 11 stitches.

Employee was attempting to slide a piece of I-beam off a concrete slab onto a forklift and placed his right hand under the webbing of the I-beam. The lower end of the I-beam dropped approximately 4 inches jamming his right ring finger between the I-beam and the edge of the concrete slab.

Employee was injured while removing metal screen from salt processing plant. The screen was stuck in position and he jerked on it and felt pain in his right wrist. He continued working his regular shift. It was finally determined on 7/27/10 that his wrist had a minor fracture and a cast was applied, forcing restricted duty.

Employee was hooking up a welding trailer to a mine tractor. The jack was lowered to its lowest point and the pin was pulled on the jack when the trailer went forward the hitch missed the ball and the trailer went to the ground on top of the employee's right foot.
