Employee was pulling a drawer of coal from the riffler with their left arm and felt something pull in EE's left upper arm/shoulder.

While lowering a section of floor grating into a work platform frame in the shop, the Welder's finger was caught between the frame and the grate. The contact cut through Welder's leather gloves and resulted in a laceration requiring 6 stitches.

The employee was assisting with a shovel move. The employee and three others were preparing to place the cable in the mule when the employee felt a pain and pop in right calf muscle.

While attempting to reposition a small section of chute liner, the liner came to a stop and the mechanics gloved hand slid along the bottom edge. The liner had been worn down to a sharp edge and cut through the employees glove and hand resulting in a 2" laceration. The employee received stitches to repair the laceration.

While moving electric shovel cable, employee was struck by the cable in the left ankle. Employee was watching the cable and the pickup in front of EE when a section of cable contacted the employee from the side resulting in a fracture to the left ankle which required surgery to repair.

A Dragline Oiler was moving a toolbox and pinched the tip of his finger against the toolbox and the hard surface of the swing case. The contact resulted in a fracture on the tip of his finger. He was wearing gloves at the time of the accident.

A dragline oiler was next to the tub unhooking a cable strap. The dragline rotated and he was struck in the shoulder with a hook shoe. A physician allowed him to work his normal duties until a surgery could be scheduled.

Employee was mounting the right hand steer tire on a 24H blade. He bent over to stand the bead seat band up on end. As he began to stand back up straight, the employee felt a sharp pain in his lower back.

Employee was manually moving a dragline swing gear bearing retainer weighing approximately 150 lbs. when his middle left finger became pinched between the retainer flange and a walkway toe board. This resulted in a laceration to the top of his finger requiring 15 stitches. He was wearing heavy leather welding gloves at the time.

A mechanic was attempting to pull a gear across a work table toward a crane. The gear slipped off the table pinching his finger. The wound took 3 sutures to close and he was given a full duty release. He returned to work on that same shift.

Employee was helping a mechanic tighten a loose hose connection in the back of a lube truck when the mechanic bumped the truck rear door lever causing the door to come down and strike the employee across the back area resulting in two (2) broken ribs.

Employee was changing a park brake on a haul truck when the bolt he was using an impact on broke allowing the brake housing to pop slightly outward and strike the top of his right middle finger. This resulted in a 2mm fracture to the fingertip and a laceration to the side of the finger that required 4 stitches. He returned to work full duty on his next scheduled shift.

The employee was placing poly pipe over the high wall when the pipe came loose from the chain connecting it to a pickup truck. When the pipe came loose it struck the employee on the ground and knocked him to the ground.

Employee was installing a belly pan on a track dozer. When he pulled back on the belly pan trying to get it in to place the belly pan pop up catching his thumb between the belly pan and the hinge on the body of the dozer.

As the employee placed his safety glasses on, the plastic lens broke in the middle causing the left half to pivot inwards striking the employee's left eye. The style of glasses has a single plastic piece for both the left and right lenses and the bridge. The glasses broke in the center of the bridge. The injury was treated by application of prescription medication in the eye.

A dozer operator was pushing a cable tower that had cable strung on it. The cable became draped over the dozer arm. In attempting to lift the cable off the dozer arm, he felt a "pop" in his left arm. He continued to work his next shifts, per ER doctor release,until he could see an orthopedic surgeon.He was diagnosed with a torn bicep tendon.

Employee was pushing a wheeled dumpster through the shop when his left pinky finger was caught between the dumpster and the tire of a haul truck that was parked in the shop. The finger was lacerated and required sutures to close.

Employee states that she was closing the cab door on a drill as she exited when her left thumb was pinched between the door latch and the latch striker mechanism. The thumb sustained fractures, lacerations and an avulsed nail.

Employee was welding in the drum of a dragline. He just completed his job picked up his tools and was walking out of the dragline to go to lunch . He was wearing his safety glasses with side shields. As he stepped out into the light he felt something in his eye. He was taken to the eye doctor where a small piece of metal and a rust ring was removed from his eye.

A rubber tire dozer operator was setting at the shovel waiting to do some work. There was a ceiling mounted heater in the cab, with a defusser/guard missing. He reached his right hand into the hole to see if the filter was missing also. The tip of his middle finger contacted the fan and was lacerated. Six sutures were required to close the wound and he was released to full duty.

Mechanic was changing hoist ropes on a P&H 2300 shovel in cramped quarters. When he attempted to pull the bucket end out of the socket, the rope dropped down pinching his middle finger. He sustained a fracture to his fingertip.

A plant employee was helping lift an idler back into position on an overland conveyor. When it wouldn't fit, they lowered it back down. One employee dropped his end causing the other employee's finger to be caught between the idler and a railroad tie on the ground. His fingertip was fractured and lacerated. The finger was put in a splint and received 4 sutures.

EE was purging propel lube lines on a 1570 Dragline. Contractors were working above him & accidentally knocked over an open bucket of EPA 2000 Solvent which in turn fell on top of him. He was correctly following all safety procedures put in place for this project. He suffered a chemical burn on his left eye which required prescription medication.

A shop attendant was securing an access ladder to an engine area after he had washed the area for a PM. He was in the process of pinning the ladder up. He released his grasp on the ladder to grab the pin. The ladder swung down and lacerated his right ear lobe. The laceration required 7 sutures to close the wound.

Employee's hand was pinched between lower pin on tow hauler grab hook and a jackstand. When the employee was removing the pin, the grab hook unexpectedly rotated causing the pinchpoint. The employee's hand received a two inch laceration that required five sutures to close.

Employee carried a floor jack to a concrete apron outside a utility shed to jack up a pickup. When he reached the pickup, he dropped the jack appoximately 1 1/2 feet to the ground. The jack handle sprung upward striking the employee's left ear. The blow caused a laceration that required sutures to close.

Employee was welding cracks on a 8750 revolving frame in inclement weather, after the shift was over and he had left the mine site. An hour and half after the shift ended he reported to a shop supervisor that he had a foreign object in his (L)eye.

The EE was trying to remove a door panel that was stuck. When it came loose, his hand came into contact with a sharp edge, resulting in a laceration to his finger that required medical attention (3 stitches to a finger).

Employee was attempting to raise the mast on a stadium light plant. When the employee got the mast vertical, he then attempted to extend the mast to raise the lights to desired height. At this time the dead end of the cable bracket broke free striking employee on the chin causing a laceration requiring 10 sutures to close.

Employee was working in the hell hole of a D-11 caterpillar dozer. To remove a breather from the transmission. When the breather broke loose from the transmission. It caused the employees right arm to move up and back striking a fire suppersion nozzle. Causing a fracture to the right elbow.
