Showing all 3Employee was working on installing a screen in one of our screener. He Claims he was struck in head with the rubber mallet that bounced off the screen after the other employee hit the screen. They were installing the bottom level of the 2 level screening deck. All PPE and lockout procedures where followed.
The employee was assisting in pulling a new electrical line for our well. He was bent over and when he stood up his back was tight. No one else was injured. He was wearing all of his PPE.
