Showing all 8Installing supplemental roof. Entire crew was rolling an I-Beam along the mine floor on the count of three. Beam rolled on top of right foot.
INDIVIDUAL DID NOT REPORT INJURY UNTIL Feb 18, 2015. 6 to 8 men were lifting 8" X 25' I-Beams to support the roof. Individual was located at the end of the beam and while lifting it above their heads he felt something "pop" down low in stomach/groin.
While lifting/hanging miner cable, felt a pull in lower abdomen.
While fixing ventilation curtain underground, pulled on curtain and injured left shoulder.
While escorting MSHA on E01 Inspection (Operating fan/airlock) opened airlock door, the inside door blew open and caused the first door to slam shut and smash middle finger of left hand causing a laceration.
