2 mechanics were reinstalling a plant crusher accumulator. The part had 3 sheared bolts and broken seal causing a hyd oil leak. EE was lifting the part back into position by hand while waiting for the hyd jack to hold it in place. Accumulator got in a bind with the support clamp and fell back to the deck smashing EE's left middle finger. EE was wearing gloves. Part weighs 30-40lbs.

Showing all 11Employee was assisting to move a battery box. EE had opened the lid of the box and braced it to check the forklift fork position. While looking into the box to check fork position, the box shifted and the lid fell down striking the left hand. This resulted in a two inch laceration that required stitches. EE was taken to the emergency room and Quarry safety personnel were notified.
Mechanic was servicing a bearing cylinder & shaft-approx weight 50lbs. It was secured in a vice that was loosened to reposition the bearing. The bearing slipped. Mechanic tried to catch it causing the EE's right thumb to get caught between the vice stand & bearing. This resulted in lacerations & a broke bone. EE was taken to the ER and treated/ released.
Employee was removing a pressure switch, when Employee released switch, a plastic part of the switch flew and hit EE's right middle finger.
While employee was servicing chains & opening doors, left pinky finger got stuck in the doors.
Employee was checking engine oil dipstick, felt sharp pain in lower right side of back.
Worker was picking metal out of concrete feed. Felt something in left eye.
Worker was trying to remove water canon hose. Hose wire cut thumb.
Employee was sorting punch plate screen panels. Panel was dropped on uneven ground which caused it to bounce back, hitting employee on the shin.
Trying to line up the bobcat tire to the studs when it slipped pinching right middle finger and between the stud and rim.
Lifted a rock to move it out of the way. Hurt back.
