On 1/5/2025 employee was lifting a lock ring for a haul truck and reported hearing a pop in EE's left arm. Employee received an evaluation on 11/5/25 at BMGH and 11/6/25 at HGH and a full release to work both times. On 2/19/26 employee reported that EE's left arm was hurting and an appointment was made for 2/23/26 with a specialist and placed on restricted duty.

Showing all 34Before the end of shift on 1/8/2026 employee was putting all their tools away in their suitcase. As they were lifting it to put the suitcase full of tools on the cart, they leaned too far and felt a sharp pain between their stomach and groin. They reported the incident to their supervisor on 1/9/2026 at the beginning of shift.
During Technical Ropes Training the employee was beginning to repel of the edge when their knee gave out.

The IP tried to control the falling pipe during the process of placing sounder pipe in well. The pipe made contact with IP left middle finger tip after it slipped out of pipe already lowered in hole, Causing an avulsion and fracture finger tip after falling 6inches.
The employee was pulling and pushing on 8 and 12-inch pipe and valves to line up the pipe for fusing. On 10/21/21, the employee reported swelling and discomfort in the lower abdomen. The employee was transported to BMGH diagnosed with a small hernia. On 10/27, the employee requested a follow-up at the clinic.
Employee was moving a shovel cable loop when they felt a pop in right calf.
Employee was digging in frozen drill cuttings for a sample when experienced pain in neck and back. Reported to supervisor on shift and requested medical evaluation.

While off loading a trailer a co-worker pulled on a pallet, containing sacks of aprox. 300lbs., with a pallet jack. This caused another pallet to shift resulting in a top sack falling onto the injured workers back.

Welder was using a dye grinder to clean up a weld when he started feeling something in his left eye. The employee washed out the eye with a saline solution for 5 minutes and still had the same feeling. The Employee stated that he was wearing full PPE, including a face shield.
