Showing all 35Employee was Tightening a flange on an ozone gas line when they received a burst of ozone gas to the face.
A forklift operator had loaded a supersack bag (1 ton) on a pallet. A coworker later noticed the bag was leaning and attempted to straighten it up by pushing on the bag and felt a sharp pain on the left side of EE's neck and had to seek medical attention. The employee was treated at the ER and taken out of work.
EE was lifting 10 pound pipe when EE felt a pull and pop in right wrist on 1/13/25. At the time it required no medical attention. The company was made aware on 8/5/25 that EE requires surgery for injury.
Employee was changing out a bullet screen on a slurry tank car and the screen slipped and in a effort to catch it, it caught EE's right pinky finger, smashing it requiring EE to go to the ER for medical evaluation.
Worker was moving another company's scaffolding for said company while wearing gloves and a sharp area on the pieces of the scaffolding's frame cut through the glove as well as the worker's hand. Witnesses were present and the worker required stitches.
Employee was washing #6 filter and was attempting to move the washup hose around and the compression clamp on the hose cut their right wrist.
EE was cleaning out from under the #4 Feeder and a lump of clay fell from top of feeder and struck the employee in the back of the head. The employee fell forward and hit the front of head.
While removing a length of frozen pipe. The pipe slipped and pinched EE's left thumb between the pipe and a block wall. X-Rays were taken and a hairline fracture was seen next to the knuckle. The miner was given a splint to wear to reduce movement.
Employee was trying to hook up an airline hose to the air lance and while doing so, they slightly slipped and their arm twisted and they heard a pop in their right arm/shoulder.
Employee was a washing a filter with a high pressure water hose when the hose came loose. Employee reached to turn the water valve off with right hand and the hose came out of left hand, whipped and pinched the right pinky finger causing a lacerations.
Employee was standing on an elevated platform to open a valve, employee was standing in an awkward position while attempting to open the valve when employee felt a pain in r shoulder.
An employee was shoveling clay from behind the feeders and belt conveyers and strained back.
The employee complained of left shoulder pain and was taken to the hospital fearing it was a cardiac event. EE was diagnosed with shoulder strain. The employee doesn't remember doing anything specific to cause the pain, it just started hurting.
Employee was doing general maintenance at the crusher, employee strained back installing flashing at cone crusher. Sought medical attention on own. Continued to work normal shifts. Was not reported until 12/29/20. Visited Dr. on 12/28/20. Could return to work on 12/30/20.
Operator was cleaning out the chemical make down shoot where the bags dump out into the make down system. A lump stopped the shoot so EE began washing down with the water hose when chemical ran down EE's arm under the chemical suit and caused chemical burns on EE's left and right arm.

EMP WAS OPENING A VALVE WHEN THEY FELT THEIR LEFT WRIST POP. MEDICAL RESTRICTIONS
PRE-EXISTING NECK CONDITIONS. EMP STEPPED DOWN FROM GREASING CENTRIFUGE FELT DISCOMFORT IN NECK/SHOULDER. DENIED THIS BEING WORK RELATED TO SUPERVISOR AND TO SAFETY MGR. LATER RECEIVED DR. NOTES THAT IT IS WORK RELATED ON 3/21/2019. LATE REPORTING DUE TO INCONSISTENCY.
EMP WAS PULLING ON A METAL DOOR ON THE VALVE BOX IN FRONT OF #2 SPRAY DRYER WHEN EE FELT LEFT KNEE POP. EE CHOSE TO WAIT TO GO TO DOCTOR. KNEE POPPED AGAIN ON 12/26/18 AND EE WENT TO DOCTOR.
EMPLOYEE WAS POKING WET CLAY OUT OF THE ROLLER MILL FEED BIN WHEN EE STRUCK EE'S LEFT LITTLE FINGER RESULTING IN A FRACTURE
EMPLOYEE WAS SPREADING GRANULAR (POWDERED) CHLORINE FOR ALGAE AROUND TANKS WHEN THEY BECAME OVERCOME FROM FUMES. EMPLOYEE WAS TAKEN TO ER WHERE THEY WERE GIVEN A BREATHING TREATMENT AND CHEST-X-RAYS TO CHECK LUNGS (NEGATIVE).
EMPLOYEE WAS REMOVING A BAGGING CHAIR FROM AIR FLOAT BAGGING WHEN EE EXPERIENCED PAIN THAT AGGRAVATED AN ALREADY EXISTING BACK PAIN ISSUE.
EMP ASSISTING ANOTHER - CLIMBING STEPS CARRYING 50 LB BAG OF CMC CHEMICAL WHEN EE FELT A POP AND PAIN IN RIGHT KNEE. EE HAD PT BUT HAS NOT BEEN APPROVED FOR A MENISCUS REPAIR AND THIS WILL RESULT IN LT.

EMPLOYEE WAS HELPING CLEAN A HEAT EXCHANGER IN PROCESS BUILDING WHEN A FOREIGN BODY GOT IN EE'S RIGHT EYE. UPON MEDICAL TREATMENT IT WAS FOUND TO BE A SMALL PIECE OF METAL. IT WAS REMOVED AND EE WAS GIVEN A PRESCRIPTION OF NEOMYCIN.

EMPLOYEE WAS BACKING DOWN RAIL CARS WITH TRACKMOBILE. EE HAD HEAD BENT CLOSE TO WINDOW FRAME WHEN COUPLING JERKED CAUSING EE TO CHIP TOOTH.
EMPLOYEE WASHING FILTERS WEARING A CHEMICAL SUIT. WHEN EE REMOVED SUIT EE NOTICED A RASH AND STARTED TO WASH THE AREA AND IT BEGAN HURTING AND ITCHING.
EMPLOYEE WAS COMING DOWNSTAIRS FROM TOP OF #1 SPRAY DRYER SILO WHEN EE FELT A CATCH IN MIDDLE LOWER BACK.
Injured Employee was pulling a 1-1/2in x 15ft.cable,4ft. to attach to the back draw bar of his dozer that was stuck. As he attached the cable to the draw bar, he felt a sharp pain in his lower back.He continued operating the dozer until the end of the shift.
