Employee was lifting a battery lid up with right hand and while doing so the lid slipped out of employee's hand coming down on right pinky finger. The pinky was X-rayed showing negative for fractures and employee received one stitch in the treatment of right pinky finger.

There were two rocks that were lodged in the chute at the top of the slope. EE was trying to dislodge the rocks when one rolled back un-expectantly on top of EE's hand causing a fracture of the tip of the middle finger on the right hand. EE also received five stitches on the same finger.

Employee was holding a pinner board to brace in-between front of the man-trip and the face of a metal panel. The board kicked out and caused two places on employee's left index finger to need a total of 5 stitches.

Employee was fabricating a new gate for reclaim feeder using plate and steel pipe. When EE went to rotate the gate the steel pipe penetrated the leather glove causing a laceration to EE's right hand. The pipe had been cut with a Dewalt chop saw and the edges had not been ground off.

EE lifted counterweight felt pain in groin area on 10/11/18. EE did not report incident in any manner. EE had surgery for hernia on 12/20/18. Initially incident deemed non work related due to lack of reporting and facts. As more facts found injury was deemed work related. Lack of reporting and duration of investigation is reasoning for delay in filing of incident.
