Employee was applying the seat belt of the equipment EE was operating. As EE pulled the seatbelt it locked up and caused the employee to strike self in the mouth with the metal portion of the belt, chipping a tooth.

Employee noticed something wrong with eye on way home from work. When ee woke up it was irritated and ee notice something stuck to eye. Employee received medical treatment for this injury at an optometrist office on 10/05/18.

Employee was using the boom of a service truck to place a piece of sheet metal. As employee positioned the metal to be placed back into a storage rack. Employee reached over with one hand to move the metal and felt a "pop at the left bicep. The employee continued to work at full duty till Oct 3, 2018 when employee was placed of work for scheduled surgery

Miner was putting belts on the #9 Slurry Pump. While rolling the belt onto the sheave EE's right pinky finger became caught between the sheave and the belt. Received seven stitches to the end of the pinky finger.

Employee stated was shoveling belt in sloppy conditions. As EE was dumping a shovel onto the belt, the mud adhered to the shovel causing it to contact the belt. When EE jerked the shovel back, EE felt a pain in lower left abdomen. EE exited mine and refused transportation. EE traveled to ER and was referred to specialist. EE is off work for unspecified amount of time.

EE was removing a drive line out of truck 045 at the time of the accident. When EE was removing the drive line out of the yoke, the drive line slipped out of EE's hands and caught the left side of EE's arm. This caused a laceration to the left side arm, resulting in (10) stitches to EE's left arm.

While removing a bolt on the "lip saver" of a 993 K model loader, a bolt with a rough edge cut the glove the employee was wearing causing a laceration to the outside edge of the right palm.

Employee was removing a hydraulic hose that had been disconnected on both ends and the clamps were removed from a 785 Caterpillar rock truck. As employee pulled they felt a pain in the upper back. Hose measured 36" long X 1.5" diameter.

Employee stated was downstacking structure on rail flatcar when another structure rolled down & struck right hand causing a large knot on the back of hand. EE & co-worker were working on each side of the car. 48' structure fell about 12'18' hitting EE in right hand which was holding another piece of structure.

Miner was assisting in changing conveyor boom on a Joy 14/15 continuous miner when the boom unexpectedly shifted. The miner was caught between the boom and the mine roof injuring miners back. Injury resulted in paralysis below miners waist.

Employees stated was picking up a cinder block when another block fell off pallet striking the ring finger on left hand. EE refused first aid and exited mine. EE refused ambulance and traveled via POV to LRMC ER where EE had a X-ray and found that EE received a broken fourth distal digit. EE was given 3 days off work and referred to a specialist.

Employee was assisting with a concrete pour and some concrete got in rubber boots causing a concrete burn (contact dermatitis) to the skin on right upper foot/lower leg area.

Employee stated was assisting setting the feeder. When the feeder was properly positioned the coworker lowered the feeder onto the tailpiece. At this time, the tailpiece chute suddenly moved catching the right thumb between the tailpiece and the chute. The EE applied a dressing to the wound and finished the shift. After work EE received 9 sutures and was released to return to work.

Employee was assisting 2 others in rotating the drums of the CM when they felt pain in the lower back/hip. EE refused 1st aid, waited for a short time but then exited the mine to recuperate. EE traveled home but on 2/10/18 at 4:45 pm stated they were going to RGH ER to be checked. After examination, EE was give time away until 02/16/18 or sees a PCP. EE remains off work.

Employee removed a top belt roller to make a splice in the conveyor belt. When ee turned to drop the roller ee felt a pain in right upper arm. EE received medical treatment on 3/29/2018 and returned to work on next scheduled shift on 4/3/2018 and begin physical therapy on 4/5/2018.

Employee accidentally ingested diesel fuel. The employee was diagnosed with toxic effect of chlorofluorocarbons, accidental (unintentional). This is a degree 7, classification 24 occupational illness.

Employee stated was cleaning sprays on CM. As EE stepped to the back of the CM to help a coworker, EE turned back to the head and struck the back of left hand on a sharp piece of metal causing a laceration. EE was transported to ER and received 6 stitches. EE returned to work on next scheduled shift.

A contractor Employee stated was sliding track ties under the rail when the rail dropped pinching left ring finger. EE applied a rag to the wound and refused further 1st aid. EE was transported to the surface to waiting ambulance and then to Raleigh Gen. Hosp ER where EE received stitches. EE remains off work. EE was not wearing gloves at the time.

12/1/17 EE & another EE was removing a piece of metal from chute when EE started experiencing pain in lower back. EE completed shift and filed an incident report that lower back was hurting, EE worked the following shift. EE sought medical attention on 12/2/17 and given full release. 12/7/17 went to different hospital and taken off work. Pain management for back since 8/2/12.

Employee was pulling CM slack into heading for left crosscut when the slack fouled causing them to be pulled backward onto their left knee. After throwing the cable down EE tried to stand and felt a pain in the left knee. EE was transported to surface to ambulance and to hospital. EE was referred to ortho dr and remains off work at this time.

The employee stated that while replacing a mechanism beam on a coarse refuse screen, the became lodged against a screen side board. While trying to dislodge the beam left index finger was caught between the beam and sideboard resulting in a contusion and laceration requiring 7 sutures. EE next scheduled shift was 11/27/17.

Employee was dragging a roof bolter cable on right shoulder as EE was going through a set of fly pads. While using left arm to raise the fly pad EE struck a roof bolt plate with left elbow, causing a puncture and small laceration.

Employee felt a pop in right bicep muscle while holding to a conveyor top-cover post on the conveyor belt to assist self in getting off conveyor belt structure. Employee continued to work until 10/13/2017 when surgery was performed to repair a torn bicep muscle.

The D-10R dozer was being disassembled in preparation to be moved. The push-arms that had been removed were sitting on the ground. Employee stepped over one of the push-arms and it fell, striking right leg causing a fracture.

Employee was working on bucket teeth and adapters on a shovel bucket. When attempting to remove the G.E.T. lock out the keeper pin fell out and pinched employee's hand between the keeper and a 2 inch plate. The plate was being used to press the wedge up using a ram and hydraulic pump. Injury required stitches to close the wound.
