At 11:30 am Employee reported that while moving the forks on the forklift they felt a pain in right shoulder.

Showing all 19Removing a driveshaft and drive tip from a capsten fitting to a hopper. The employee straddled the driveshaft with left hand holding the drive tip and right hand was holding the driveshaft. The employee out stretched arms could not control the driveshaft and drive tip, causing it to fall 18" to the concrete. Employee left hand middle finger was pinched causing a laceration.
While installing a steam rig on the bottom of Tk 48 employee stepped in residual ore/liquor which seeped into the bottom of EE's boot through a hole and burned a small area on EE's left lateral side foot.
Employee finger caught between wrench and top of freight elevator door. Causing fracture to the finger tip and laceration to finger requiring stitches.
Employee was walking into Press Floor area at NE entrance to assess scale buildup on wall to determine work that needed to be done in area. Was in process of communicating to mobile equipment operator supervisor when liquor dumped on back and neck from floor above. Caustic burns to upper back, posterior neck, upper anterior and posterior of left arm, small spot on left wrist
Personnel had rolled spool out of #1 Decanter U/F Pump and was washing rocks out of pump, this loosened plugged material in pump causing hot caustic mud to spill out of discharge line. Personnel heard material break free in line and turned to get away from it when it came out of pump across majority of back. This caused burns to back.
Left forearm foreign body.
Employee was pushing forward to open a valve using force and EE's forearm made contact with a bolt underneath the valve. Resulting in a laceration to EE's (R) forearm that required 4 stitches.
Powerhouse boiler operator was climbing steps (2) at a time going to restrooms. When the operator felt a pain in right shoulder.
Employee was opening a three way valve and caustic made contact with their left eye and both nostrils.
Employee was removing the wrapping from around a new light stand, and when ee loosen the stabilizer bar finger was pinched between the two bars.
Employee was in precipitation standing on a platform at Tank -52. Removing a 90 degree steam injecting piping. When they removed the last (2) bolts they felt caustic liquid enter their left glove resulting in a burns to the fingers.
Employee was blasting a metal plate when the air hose was entangled on the plate resulting in a left shoulder strain.
Employee was turning the belt pulley on the east mixer to see if it would turn freely when ee crushed 4th finger on left hand.
EE taking motorized pipe beveller out of storage locker. Felt a pull in left arm. Started to swell went to medical, evaluated ice applied to area and RTW. Follow up visit to medical, EE sent to Orthopedic doctor 10-17-17 for evaluation. Orthopedic diagnosed rupture left bicep.
Employee was putting water on the suction of tank Y-5 to fill. After turning water on EE approached suction ball valve to check if water was flowing and at that time the hose came off from the Chicago coupling. Employee had caustic spray EE in the face EE was wear chem goggles spray entered goggles and contacted eyes.
Motor was being lowered onto vertical pump shaft, choker cable broke motor dropped down vertical pump shaft approximately 3 feet. Employee was hit on brim of hard hat caused laceration over left eyebrow and abrasions on left hand and lower left leg.
Employee opening #11 filter press, press had not competed draining and caustic (liquor) came out when the press was being backed up. Caustic (liquor) sprayed employee lower legs. Employee sustained caustic chemical burns to lower leg area.
Employee was moving a piece of metal plate on a shop table. While doing so, the employee felt pain in abdomen.
