Debris entered eye requiring medical treatment to remove.
EE was installing 42" pipe when the wind blew into the pipe causing dust and debris to enter EE's left eye. Seen at eye doctor on 4/23, diagnosed with a foreign body in the eye which was removed and prescription eye drops given. Released to full duty on next shift.
EE was assembling core shed table. Putting bolt and nut on, had left hand on table holding bolt. While pulling down on the table, felt sharp pain in left side of chest. Seen at clinic, diagnosed with a pectoralis strain, released to light duty.
Ee had climbed up into EE's haul truck and sat down. EE dropped a paper, pulled the arm rest up to reach the paper, when EE felt a pop in the right side of EE's right shoulder, then instant pain. Seen at ER, diagnosed with a right proximal humerus fracture. EE has been diagnosed with osteopenia which can lead to non-traumatic fractures.
EE was dropping bolts off at headings underground. A bolt slipped out of their hands back onto the truck, and the bottom of the bolt hit them in the face, cutting their cheek. Seen at ER, wound closed with sutures, and the EE was released to full duty.
Miners were attempting to remove an old pump discharge pipe. The upstream valve had been isolated. While loosening the 4-inch Victaulic fitting, the pipe separated due to tension within the line, causing one of the pipes to swing toward the employee, striking EE in the right lower chest.
Ee was changing an impact slide on stacker #2. Impact slide fell, smashing ee's pinky. Seen at ER, xrays negative for fracture, wound closed with sutures, and ee released to full duty.
Ee reports pulling on diesel hose after fueling up equipment, feeling sharp, twisting pain in abdomen. Finished fueling, after about 20 minutes they threw up. Continued to fuel island, parked, went to blue room, threw up. Reported to lead, transported to ER, diagnosed with hernia, referred for surgery. Working restricted.
EE tripped while carrying a box of booster. Reached out with left hand/arm to break fall. Seen at ER, diagnosed with possible fracture, referred to ortho. Ortho appointment on 1/21 confirmed fracture. EE working restricted duty.
The injured employee was installing Purlin on the Ren Shaft Hoist House when the Purlin rolled, the employee reached to stop it and was cut on the left arm/wrist above the glove.
While removing starter from loader, part slipped, ee tried to stop it from falling, it caught on EE's glove, pulled EE's arm, causing pain in ee's left shoulder. Seen at ER, diagnosed with strain. Follow-up at clinic confirmed strain, referred to orthopedics and for MRI, released to restricted duty.
EE was changing out water pump when water from the pump splashed them in the face. They continued to work, finishing out the shift. The following day, they went to the clinic, was diagnosed with bilateral conjunctivitis. Released to return to work full duty.
Ee reported to company on 12/12 that on 12/7 EE was lifting and moving a spool of wire repeatedly throughout the shift. EE reports feeling pain that day and the next day. On 12/11 EE went to ER. On 12/15 EE filed a claim for workers comp for the incident, and on 12/19 it was accepted by workers' comp.
EE reported a rash on legs, seen at clinic on 12/2, diagnosed with rash. On 12/23 follow-up, diagnosis changed to chemical burns, EE put on restricted duty, making this reportable. Site offering temporary modified duty.
EE was cleaning a lime discharge chute. The chute went positive, and lime shot into the EE's left eye. Seen at ER, diagnosed with chemical burn to left cornea, transferred to eye specialist. Released to full duty 12/11.
EE was removing wire from conveyor belt. While pulling wire with force, the wire gave way, ee fell back onto metal stairs, hitting head and neck. Seen at NNRH and then Health. Released with restrictions until 11/1/25, when EE is released to full duty.
Ee dropped drill bit. When EE lifted it back up, EE felt a pop in EE's right biceps tendon. Seen at clinic, referred to ortho. On 12/5, ortho confirmed MRI results show full tear of right biceps tendon, and ee scheduled for surgery on 12/30. Ee working restricted duty.
Ee was throwing away a piece of hardline. When EE went to push the hardline over into the dumpster, it slipped and crushed EE's hand between the hardline and the Dumpster. Seen at the clinic, released to restrictions, and referred back to orthopedics.
On 10/17 ee working with other ee unloading cement chute from pickup. While moving it, ee felt cramp in lt arm, and coworker reports ee "exclaimed loudly" and let go of chute. Ee said EE felt pain in EE's arm. Ee did not report this to site until 10/23. Seen at clinic, diagnosed with possible biceps tendon tear, referred to ortho.
EE was putting a gold bar into the cleaner slot and they caught a finger between the cleaner slot and the gold bar. EE was seen at health, diagnosed with a crush injury to the left middle finger, and released to modified duty, no use of the middle finger of the left hand. Site offering modified duty, EE currently working.
Worker's kneecap popped out of place when trying to sweep an electrical cable to the side with EE's foot
Ee was showering after work using Dawn dish soap. Soap fell off shelf, ee tried to catch it, squeezed soap in eye. Seen at clinic, diagnosed with irritation, put off work. Seen at Total Eyecare the following day, diagnosed with abrasion to right eye, put off work until full duty release on 9/30.
EE was leaning on rail while wiping a mirror on a piece of equipment. EE reports that they were leaning on the rail in order to wipe mirror down low. Felt their left rib pop and reports a deep breath hurting. Seen at ER, diagnosed with rib pain, and released to full duty. On follow-up, released with restrictions site cannot accommodate.
A heavy toolbox being moved on fork tines fell to the ground. Two employees attempted to manually stand it upright. During the lift, one employee lost control, causing the box to shift and trap the other employee's finger between the box, the ground, and EE's boot, resulting in a laceration. (requiring several sutures)
Ee helping move brassboard weighing approx 200lbs from lineout room to the rear of flatbed truck. During team lift, in process of lowering brassboard onto flatbed, ee's left index finger was pinched between edge of brassboard and a toolbox on the flatbed. Seen at clinic, referred to ER for ortho consult. Fracture, released to full duty.
EE was chipping concrete while in a batch truck. After they got out of truck, they felt stinging sensation on buttocks. They had somehow gotten cement material in underwear/under clothing causing burn to buttocks. Seen at Health, referred to ER for removal of cement. Released to restricted duty, working restricted duty.
Driller assistant sustained a finger laceration to left index finger that required 9 stiches to close. The injured employee was manually handling a drill hammer and was attempting to set it down on a block of wood to repair when it slipped from their grip, causing the finger to get pinched between the driver sub and the hammer bit.
EE was cleaning the inside of light vehicle. While wiping down the back of the seat, a wire poking out of the back cut their right hand. EE was seen at a clinic, diagnosed with a laceration to the right palm, closed with stitches, released with restrictions that do not preclude full duty.
EE was removing a schedule 40 elbow while standing on scissor deck. The elbow came down on their hand, pinching left ring finger. EE was seen at the ER where the cut was sutured and EE was released to full duty. The EE initially reported that they tripped and fell, but this was untrue. EE issued an amended statement on 7/30/25.
EE walking up stairs at the mill, carrying a flat of water. Felt a pop in knee. They saw dr, released to full duty. EE requested follow-up as it was not getting better. Following MRI, on 10/09/25, found ruptured ACL, making this reportable. EE scheduled for surgery 11/13/25.
Ee was lifting a piece of pipe off a pipe handler and EE felt a pop in EE's left shoulder. EE felt immediate sharp pain. EE was seen at the clinic, released to restricted duty, and referred for an orthopedic consult with suspected rotator cuff tear.
Ee was at the 1600 fuel bay. Lifted the fuel hose to attach to truck and felt a pop in EE's chest and right side rib area. Seen at ER, diagnosed with a chest wall strain, possible tear of pectoralis muscle. Released to restricted duty.
The IP was abandoning the hole and stepped into the drill hole "cellar" to finish the cement cap. As EE did, EE's knee popped, and it began to swell; EE couldn't put weight on it. Was treated for sprain and edema to the right knee and found fit for duty with restrictions. Kneeling, squatting, twisting, jumping, running, and climbing ladders.
Ee was moving auger and pinched right ring finger between auger and fitting on the pug mill belt. Was seen at ER, diagnosed with open fracture, referred to orthopedic hand specialist, released with restrictions.
Ee was emptying a dumpster. EE released the latch on the handle of the dumpster and it flipped over and pinched EE's hand. Seen at clinic and referred to ER. ER referred EE to ortho, seen by ortho and surgery done on 5/20. Released with restrictions and site offering modified duty.
Ee reports about 3/4s through shift feeling irritation in right eye. Flushed eye, didn't notice anything on rag after drying eye. At end of shift, eye was very red. Woke up next morning, eye was crusted and puffy. Seen at Golden Health, diagnosed with conjunctivitis, given antibiotic eyedrops. Released to full duty.
Coworker closed pickup tailgate on ee's hand. Seen at clinic, xrays negative for fracture, released with restriction that did not preclude full duty. Ee seen on 6/27, released to restrictions that do preclude full duty. Site offering modified duty. Ee referred to hand surgeon.
Ee was operating scissor deck. EE looked up, and something fell in EE's eye. EE reported it immediately and requested to be seen the next day. Seen at Total Eyecare, diagnosed with an abrasion to the rt eye with mild inflammation, given steroid eye drops, and released to full duty.
Ee was function testing the jackleg when a piece of drill steel popped out, falling on the ee's foot. EE reported the incident immediately, but did not seek treatment until 4/28. Xrays positive for fragmented fracture of the right 5th toe. Referred to ortho and released to restricted duty.
EE reported reaching up and pulling bag of rags weighing approximately 25 pounds and felt pain in shoulder. Seen at clinic, diagnosed with strain, released with restrictions. After medical and factual investigation, workers' comp accepted claim on 4/17.
Ee felt pain on left side of groin after lifting an oxygen bottle on a cutting torch into the back of a pickup truck. Seen at Health, referred to ER for confirmation of hernia. Ultrasound positive for hernia. Referred for surgery. Ee released with restrictions site can offer modified duty for, then termed for cause unrelated to the incident effective 03/31/25.
Ee was removing paste pipe. While removing the chain, the pipe moved, pinching the ee's hand between the chain and the pipe. Seen at Health for a 4 inch laceration to the hand, referred to hand surgeon. Ee released with restrictions site can offer modified duty for.
EE was attempting to close the valve on frac tank after they filled up the pressure washer. They encountered some resistance when trying to close the valve so they used both hands to shut the valve off and pinched their middle finger between the valve handle and lock pin handle.
Ee pulled the top door mechanism of the hot change bus to close the door and pinched finger against the rod. Seen at ER, diagnosed with a fracture to the tip of the pinky finger, and released to full duty.
Ee reported ee's back feeling tense ee's entire shift. When ee moved a sample bag towards the end of ee's shift, ee felt a "crack" sensation. Went to break, felt "warm sensation" and couldn't move. Went back to work, pain got worse, reported. Cumulative tense feeling in back cumulative in nature.
Ee lifted a pump weighing approximately 30lbs and carried it. Ee reports feeling a "thud" in ee's back and having back "lock up". Ee reported this to workers' comp on 3/10 and was seen at the clinic on 3/11. Diagnosed with sciatica and a strain, aggravating a pre-existing work related injury. Released with restrictions.
Driller was attempting to add brake line antifreeze to the compressor and had removed plug when the driller inadvertently opened the valve and residual antifreeze in the line sprayed driller in the face causing some antifreeze to enter eyes.
Ee was cleaning a jammed piece of copper from the stripping machine. Wearing gloves and using finger to try to remove the jammed piece and felt a cut. Took off the glove in the restroom and left index finger was cut. Seen at clinic, diagnosed with a laceration that was closed with 4 sutures. Released to work.
