Showing all 4EE was pulling on a V-belt while installing it onto a pump motor sheave. When the V-belt slipped on to the sheave, EE's right hand middle finger got pinched between the belt and the sheave. When EE pulled the hand back, the tip of the middle finger from the nail bed forward was cut off.
Employee was installing a conveyor belt take up pulley. While pushing the pulley into position, the pulley fell and struck the employee on EE's left arm and ankles.
