Employee was discarding sand samples into a 5 gallon bucket for disposal back into a sand pile. The employee was in the process of throwing the discarded sand back into the trash pile when they felt a pop in their leg area. Employee was sent to clinic for eval. First aid pulled muscle was diagnosed. Went back 1/14/26 for eval and has hernia.

Showing all 7Employee was stepping into a Bulldozer when the injury occurred. While pulling self up into the cab of the dozer, the employee felt a pop in right arm. The employee went to get the arm checked out the next day. On 9/3/24, employee was able to get an MRI and this indicated a tear of the right bicep tendon.
The employee was in the process of replacing pump packing at the Wet Plant Pad 4. When returning from truck with packing the employee stepped down from a concrete retaining curb and took approximately 3 steps and lost their balance. The employee slipped on the concrete pad and landed awkwardly on their leg. This resulted in the employee breaking their lower right leg.
Vista employee was picking up debris (PVC Pipe) at the pit location when one end of the pipe snapped and struck the employee in the upper right lip requiring 5 stitches resulting in a reportable injury. Employee was able to return to work without restrictions.
Vista employee notified Josey's Contractors v-belts were burning off of exhaust fan, later was shut down. Vista called Josey's to replace v-belts. After LOTO, while twisting and turning the belt into place when left handed index finger got caught between belt and equipment.
The miner opened a new can of welding rods but did not remove the lid completely. EE then carried the container to the bed of the truck, as EE sat the container down it started to tip over. The miner tried to move EE's hand away from the container causing a laceration on EE's right palm requiring sutures. Miner was not wearing gloves at the time of incident.
Miner was carrying a cardboard box of trash to place in pickup truck bed. Miner lost footing and fell into the tailgate of the pickup.
