EE was at SD-1 Substation skid placing a 480VAC breaker into cubicle. EE backed truck into position approximately 6-8 ft. from cubicle w/breaker in the box of truck. EE carried breaker to the cubicle, crouched down to insert the breaker into cubicle & felt burning sensation/pain in left arm.

Employee was working on P5 outside the Pellet Plant, while closing junction box cover, EE felt something fall behind safety glasses in left eye. Wind was blowing ~10-20 mph. Evaluated 4/3/24 & issued antibiotic drops.

EE on L2 Roll Conveyor was installing new outlets. EE reported they had looked up & dust fell on face, hardhat, in eye. Later that night, EE experienced left eye irritation. Removed contacts out & flushed eye. Reported incident to supervisor on 3/21/24. Evaluated 3/21/24 & issued antibiotic drops.

Hibbing Taconite Company · Caught in, under or between a moving and a stationary object

B.O. elec. pallet jack moved w/strap & pickup truck. Unit wheels contacted gravel causing to tip w/forks perpendicular to ground. Unit put in correct position, pushing down w/hands on back of pallet jack forks. EE's left hand was on fork load wheel. When unit drive wheel contacted ground, load wheel receded under pallet fork, pinching EE's left fingers.

EE was changing out empty Oxygen bottle on hand cart. While moving old bottle off by tilting it forward and lifting slightly over lip of cart, EE felt pain in left elbow.

MMG was placing wheel chock stand from bed of truck around wheels of parked vehicle. Wheel chock handle laying down in bed of truck, rotated upward unexpectedly striking MMG on right side of safety glasses. Resulted in a cut (stitches) and bruise to right upper cheek of face.

EE was sandblasting while cleaning dust collectors on Concentrator Line 7 and reportedly got dust in their eye. EE reported incident a day and a half later due to irritated eye & sought medical evaluation resulting in antibiotics ointment for eye.

EE stepped down from conveyor belt onto block of wood. Left hand was holding conveyor belt frame while stepping down on the block w/right foot, then stepped down to floor w/left foot when the wood block tipped to the side. Left arm gripping conveyor belt frame resulted in pain to left tricep.

Employee wearing leather gloves was sliding a piece of cut 1 1/4" plate on the cutting table when the plate dropped down pinching the tip of the right middle finger between the cutting table and steel plate resulting in a closed fracture.

EE was removing large sheet metal pieces of an old HVAC unit. While pulling a piece EE lost grip, stepped backwards catching the top of their right forearm on sharp corner of a duct behind which resulted in a cut & stitches.

MMG helping to change 18" pipe in the Concentrator (line 8 Finisher Floor). One section of the 2 piece clamp was removed and placed on the rolling stair. MMNG moved rolling stair & clamp fell from the platform of the rolling stair striking MMG in the face area.

EE was climbing boarding ladder for grader on ready line, placed foot on first step, grabbed handholds, began to pull themselves up on clean, dry step from the ground & felt pain in right shoulder. Diagnosed right shoulder tendinitis.

EE was repairing lighting on top of the lube van, stepped on the first step to cab to turn on light switches to test lights. EE leaned across the cab seat putting pressure on right shin from the second cab step. Resulted in slight pain but EE continued working. EE sought medical treatment days later resulting in a contusion/hematoma. Incident was reported 4 days after occurrence.
