Clevis broke on a retriever, striking other employees. When this occurred, a jack was about to fall on top of another employee and injured grabbed the jack to steady it, when EE did EE felt a sharp pain in EE's right bicep, received surgery to repair a torn bicep on the 26th of February.

Employee was installing 20ft center bolts in 1M headgate section. As employee was changing out drill steels, they lost their grip on the steels in the roof, causing them to fall striking the top of their left hand. Employee was diagnosed with a fracture to the third and forth metacarpal.

While prying a rock off the miner, the drill steel shifted up when the rock came off the miner. They let go of the steel and it came down and hit the top of their hard hat, which pushed down on their safety glasses causing a scrape on their nose and a chipped front tooth.

Employee was carrying a beam with another employee to put on top of bolter. The employee in the back tripped on rough bottom and dropped the beam. The beam accidentally slid, and inadvertently, the injured employee's finger got pinched between the rib and the beam.

Employee was putting a high line hook of extra bits in the parts car at the load center. When EE let go of the hook, the end of the hook caused a laceration to EE's left index finger. Employee received sutures so close the laceration and treated and released to return to work. No time was lost.

Initial treatment sought on 11/16/25 with a follow up to specialist for final diagnosis on 12/1/25. Final diagnosis of pelvic and tailbone fracture. Employee was not pinned.

Employee was picking up K-doors with crew to put on the hinges. A piece of metal skirting came loose from the door and slid and accidentally caused employee's thumb to get caught between two pieces of metal.

Employee was changing a cradle at 3 East 58 Wall, 4 Mother Belt on the tight side, while the employee was maneuvering the cradle it slipped pinching the right ring finger between the cradle and the bedrail. The event resulted in a Fracture to right ring finger and a laceration requiring sutures.

Employee was tightening chains on the tires for the section scoop. While cleaning up from the job, EE moved the fairlead from the scoop and placed it on the toolbox. The fairlead slid off the toolbox and employee tried to catch it, smashing EE's right ring finger between the fairlead and the mine floor resulting in a laceration at EE's last knuckle.

Employee attempted to pull slider out of ventilation tubing and the slider did not move, employee was reaching over EE's head facing the coal face. Employee pulled harder with both hands and felt a sharp pain in EE's stomach. Employee noticed lump above belly button and has been diagnosed with a herniation of the Umbilicus.

While removing CLA bolts from a continuous miner, the bolt broke loose causing the employee to jam EE's wrist. Employee originally went to ER on 4/1 and to orthopedic on 4/4 where EE received a full release and returned to work both visits. At a follow-up with orthopedic on 4/17/2025 x-rays were conducted for a third time with a diagnosis of a fracture to the wrist.

Employee was working on the 6879 CM. EE had the light cover lifted and were working on the trailing cable junction box for permissibility purpose. An EE had turned their head to speak with another EE as that EE was walking by when the lid came down smashing IE's hand/ finger between the lid and top of the junction box cover.

On 2/4/25, a visit to the doctor revealed a previously undiagnosed fracture to the foot, and they began missing time.
