He went back to the doctor on 6-20-14 and was taken off work until further notice making his first day of loss time as of Monday 6-23-14. *Employee returned to work on 7/14/14 and went back off work on 7/21/14 as a recurrence of old injury.

Employee started to get out of supply motor and put his right elbow on top of canopy and his left hand in handle and pulled himself up and felt his right shoulder pop. Employee later required surgery and started missing 03-14-14.

Employee stated that he may have pulled a muscle in his right side while packing belt structure to scatter for the next set-up. He stated while packing the structure he felt a sharp pain in his right side that made him uncomfortable to stand, sit, or lay and the pain was getting worse. Employee later required hernia surgery and started missing 01-15-14.

A rock became lodged in the auger tube. When the employee pried the rock loose, bolts securing the auger also loosened, causing the auger to fall from the sample tube. The employee suffered crushing injuries to his right arm as a result of this incident.

Employee was replacing a blown hose on a CAT D-11 Dozer and had to drop the front belly pan in the process. The belly pan was supported by a strap which became fouled, creating slack, and allowing the belly pan to drop 6" - 8". The belly pan struck the victim in the head, resulting in a laceration that required suturing.
