The employee had exited backhoe to help make a pipe connection and was in the process of remounting the machine. After placing right foot on the step, the employee used the grab handles and right leg to step left leg onto the track. When this happened, the employee felt something pull in right leg.

On 2/2, employee flipped a metal plate (45#) from track. 45 minutes later, EE reported to Supervisor that EE may have messed back up. EE refused treatment & said was okay twice during the shift. Employee had back surgery last year. The mine received doctor's notes on 2/25 that EE's back doctor blamed pain on employee flipping plate. Employee has a stretched nerve in back.

During the process of loading up a 2-inch discharge hose some residual fluid in the hose splashed on ee face from the end of the hose and ran down behind safety glasses into ee eye. The fluid was wiped away and then rinsed with fresh water. There was no pain, discomfort or indication of injury so ee returned to work. Later that evening some irritation was noticed.

The employee was operating #310 water truck. After lunch break, EE noticed that the remote for the water fill had fallen on the floorboard on the right side of the seat. When EE reached for the FOB, EE felt something touch the tip of EE's right little finger, when EE looked down, EE noticed blood. During the investigation with the employee, no sharp objects were found.

Employee had changed oil in D10N bull dozer as employee was putting up the left side engine guard with left hand, at which time the employee felt like left arm got a cramp in it. Employee reported to supervisor that arm was still hurting after about an hour had passed. The employee was taken to the doctor and was diagnosed with a sprain to the bicep muscle in the left arm.

Employees were transferring brake calipers from one wooden pallet to another wooden pallet. During the process of moving brake calipers one of the employee's finger was pinched between two calipers.

Employee were removing tire ring from haul truck wheel. When the wheel was placed in the upright position the ring shifted. The employee reached to stop the ring from shifting, the tip of his finger was pinched between ring and wheel.

Employee was removing a pin from the dragline bucket with a hammer. The injured employee was standing approximately 15 feet behind the area when a small piece of the pin detached and struck the employee standing behind the work site in the forearm.

Employee was readjusting a lowboy outrigger board on an empty lowboy trailer. He picked up the board with his gloved right hand and then pushed with his gloved left hand and the pad of his ring finger became pinched/caught between the board and the outrigger stand.

The dragline crew was re-socketing drag ropes. While inserting the wedge, the employee placed his hands on the upper side to guide it into place. As the wedge slid into place, the pad of the employees right ring finger was pinched between the top of the wedge and bottom of the rope.

The injured employee and a coworker were walking along a dried creek bed holding a pipe that had a roll of barbed wire on it. The injured employee slipped and went down to one knee. When this happened the wire slid down the pipe hitting him in the arm causing three small lacerations.

Connecting lift cylinder to blade on D7G Dozer, EE's hand was in path of pinch point while he was lifting up the hydraulic cylinder.

Two employees were removing the right front suspension cylinder spindle off of a 777C truck. EE was using a 1" impact wrench. He had 6 of the 9 bolts removed when the remaining 3 bolts broke allowing the spindle to fall. As it fell, the spindle rolled onto EE's left foot.

EE was holding the front arm of the 16G Motor Grader. Another contract employee was tapping a bushing in with a 3 pound hammer. A piece of the bushing broke off and punctured EE's left arm (inside elbow).

Employee was pulling slack on 3/4 reaving. Lost his grip and cable came up and hit in the groin area. This made him sick at his stomach and he threw up. He appeared to be over heated as well. Employee was moved to a shaded area to cool down. He was then taken to the hospital for evaluation and received IV fluids. Employee was back to work next shift.

Employee was placing 105 lb. Spreader Bar & Dump Block pins into pin hole and the pin slid into place faster than expected crushing employee's left tip of ring finger between the pin head and the dump block..

Dozer operator was dismounting dozer, when he stepped backward off dozer blade arm, he felt pain in groin area.

Working on D7E SCG115 in area N. Task was to inspect for an oil leak. Lowered belly pan under front lower radiator. Belly pan bolts snapped under weight of mud collected in pan. Belly pan fell on Kevin's right knee, caused a sprain to right knee and ankle.

After placing a trash bag into a drum, the Oiler pressed down on the plastic bag to deflate. A small wire pierced through the bag and his glove. It made a small mark on his middle right finger. Two days later he experienced pain and swelling in the finger. Treatment for infection was given at the local ER. No work restrictions were given to employee.

The employee was relocating an empty aluminum crate when the incident occurred. As he released the crate, the back of his middle finger came into contact with the aluminum ID tag. He received a minor cut on the back of middle right finger. It was determined by the ER doctor that three stitches were required because of the location of the cut to the first joint of the finger.

Employee was cleaning up around the shop when he needed to move a rolling toolbox to sweep under it. As the mechanic was moving the box, it rolled into a building column, striking his left thumb. The employee was taken to the ER where he was diagnosed with a small fracture to his left thumb. His thumb was cleaned and he was released.

While removing a drag drum bearing cover the cover slipped causing the employee to twist his back. MRI received no disk damage. A strained nerve is the diagnosis.

During a grinding operation, a welder removed his full face shield wiped his forehead with his shirt sleeve. He felt something in his left eye. He was still wearing safety glasses. The eye dr removed an embeckled small metalic particle from his eye.

While performing a preshift inspection the hed pulled on the plastic hood latch, the handle broke cutting his finger.

EE pulled out a rear idler on D10N-6 & had removed the new idler from the crate. He then lowered the old idler into the crate & removed the lifting straps. He then tried to lift the crate w/the used idler, when the crate failed & the idler fell onto his left ankle.

The injured installed a used battery on a 6" water pump but did not clean/remove corresion from the battery posts. a co-worker turned the key to start the pump. At that time an arc occurred and the battery top failed and the battery cap and other debris flew up and struck him.

Injured was repairing a leak in a discharge pump pipe near a highwall. The pipe was chained off to keep it from going over highwall. The coupler was removed from two sections of pipe and when the injured pulled on the pipe to separate them, the pipe sprang upwards, striking the injured in the upper lip.

EE stated that he was lying on the ground changing a fuel filter on a chevrolet pick up when 2 foreign body fell into his right eye. He said he flashed it out with eyewash and continued working. Two days later his eye become painful and felt like something was in it.

EE WAS INSPECTING 480 DRYER AND GRABBED ONTO THE BELTS TO REPOSITION HIMSELF WHEN THE BELTS MOVED, PINCHING HIS FINGER BETWEEN THE BELTS AND PULLEY.

EMPLOYEE WAS KNEELING DOWNM BENT OVER, TRYING TO PLACE A NYLON STRAP ONTO A SEVEN (7) INCH DIAMETER PIN. WHILE TRYING TO ROLL THE PIN ONTO THE STRAP HE FELT A PAIN IN HIS BACK. EMPLOYEE DID NOT MISS WORK DUE TO THIS INJURY UNTIL MAY 27, 2004

Was lifting a section of track rail to place in scoop car with helper and hurt left rib area.

EE WAS HANGING CONTINUOUS MINER CABLE AND TWISTED HIS KNEE. HE STARTED MISSING WORK 4/13/04.

HE WAS PULLING ON THE SHUTTLE CAR CABLE, WHEN HIS BACK WENT OUT.

HE WAS TRYING TO GET A ROOF BOLT INTO THE HOLE & PUSH ON THE ROOF BOLT. HE CLAIMED THAT HE HURT HIS WRIST. HE DID NOT BEGIN MISSING WORK UNTIL 1-7-04.

WHILE WORKING ON 390 SCREEN EE WAS HOLDING A CLAMPING BAR WHICH WAS WORN AND WHILE ANOTHER EE USED AN IMPACT TO TIGHTEN IT THE PLATE SPUN AND CUT HIS FINGER.

OPERATOR CLAIMS HE STRAINED HIS BACK PUTTING HIS DINNER BUCKET ON THE ROOF BOLTER.

REACHING TO GET A BOX OF GLUE AND HE FELT A PAIN IN HIS LOWER BACK.

EE WAS HELPING TO PUT A COVER BACK ON MINER, THE COVER SLIPPED & PINCHED HIS FINGER BETWEEN COVER & MINE FRAME.

EMPLOYEE WAS MOVING TOP BELT STAND. HE LOST HIS GRIP AND IT FELL, PINCHING HIS THUMB AGAINST BED RAIL.

BUGGY CABLE CAUGHT IN SHEARER WHEEL AS HE PULLED ON CABLE, FINGER CAUGHT BETWEEN CABLE & SHEARER WHEEL.

EE WAS GETTING OUT OF TRUCK AND CLOSED THE DOOR ON HIS FINGER

PULLING ON BELTLINE WHILE LOADING BELT ON FLAT CAR AND CLAIMED HE PULLED A MUSCLE IN HIS BACK.

WELDING ON CUTTERHEAD, HE HAD TO OPEN A NEW CAN OF WELDING RODS BUT DID NOT TEAR THE LID COMPLETELY OFF, HE REACHED INTO THE CAN AND CUT RIGHT INDEX FINGER ON THE EDGE. HAD TWO STITCHES BUT NO TIME LOST.

He was loading give on the the roof bolter and felt like something came out of socket. start or missing work 4-13-04

CHANGING OIL IN 234 DOZER, LET DOOR OVER FILTER'S FALL DOWN, STRIKING LITTLE FINGER OF LEFT HAND. HAD STITCHES AT PIKEVILLE METHODIST HOSPITAL.

HELPING TO CHANGE BEARINGS ON CRUSHER, CO-WORKER WAS TURNING FLYWHEEL BY HAND, INJURED ATTEMPTED TO HELP BY PULLING ON V-BELTS AND FINGER WAS CAUGHT BETWEEN PULLEY AND V-BELT. INATTENTION TO A CTIVITY MAJOR FACTOR.

REPOSITIONING SHUTTLE CAR TRAM MOTOR ON 3 WHEELER. FIRST DAY ABSENT FROM WORK WAS 03/04/2003.

HE WAS LIFTING CRIB BLOCK OFF OF FEEDER & CAUGHT FINGER BETWEEN CRIB BLOCK AND FEEDER.

EE WAS HELPING TO LACE MINER CABLE AGAINST RIB, WHEN HE EXPERIENCED BACK PAIN.

ROLLED OUT OF THE SCOOP AND WHEN HIS KNEE HIT THE GROUND IT TWISTED THE KNEE. HE HAD SURGERY ON HIS KNEE ON 02/14/03. IT WAS DETERMINED TO BE JOB RELATED ON 02/24/03.
