Employee lost control of a high pressure wash down hose while hosing the right side of the stage loader in 12L. The hose struck EE in the face causing fractures to EE's face.
Employee was removing rock from the continuous miner head when they felt tightness in their back. Employee finished their shift and informed the safety department the next day that their back was still tight. Employee was diagnosed with a back strain on 12/17/2024.
Employee was grabbing the hook of a top roller in which was halfway off of the bottom belt. Afterwards, employee felt damage to their right hand.
Employee was assisting mechanic lift a shuttle car sideboard. The mechanic needed to reposition themselves when they went to place the sideboard down the employees right middle finger was caught underneath. That action resulted in an avulsion that required 11 sutures.
Employee was offloading winder bars at the 2B Belt winder area. As EE was offloading the bar, EE felt tightness in EE's lower back. Diagnosed with a herniated disc. LTA as of 11/19/24.
A bolt broke on a chute and the chute landed on employee's right middle finger, causing a laceration. Injury was not the result of a training or compliance issue.
EE was working on the #1 North Section tramming the miner from the #7 entry to the #8 entry. While tramming the miner the standoff caught a piece of the waterline, the EE hit the side breaker and removed the waterline and pinched the middle finger on left hand between the coupling and the standoff causing a laceration resulting in (4) stitches.
Employee attempted to uncouple a mantrip by hand. They reached for a coupler and attempted to disconnect. At the same time, another employee hit the hydraulic uncoupler on mantrip and then injured employee accidently got a finger caught between two couplers causing a laceration.
Employee was rib pinning in the 14L Section, when a rock fell from the mesh and struck EE in the left ear. Employee finished EE's shift and was treated to close the laceration with 6 stitches. Employee did not miss any scheduled time as a result of the injury.
Employee was installing intersection bolts in the 7 South Left Section, when EE was removing EE's drill steels from the roof, it slipped and struck EE in the bottom lip. Employee finished EE's shift and was treated for a laceration with 3 stitches to close. Employee did not miss any schedule time as a result of EE's injury.
Employee was loading a bottom roller into the hoist car, when EE slipped and pinched right pinky finger between the roller and the hoist car. Employee received seventeen stitches.
Employee was carrying wooden posts a distance of 11 blocks and felt EE's shoulder pop. Injury was not the result of a training or compliance issue.
Employee was rib bolting on the continuous miner in the 2 south right section. EE was assisting in hanging a ventilation tube when EE felt a sharp pain in left shoulder.
EE was building cribs at break #42 outby #1 West Section and some debris got in the right eye of the EE. Note: EE went to the doctor the next day thats why I'm not reflecting a LTA.
Employee was performing a service on a microtrax in diesel shop. Employee and helper were sliding top cover back onto machine when it slid to the side and pulled employees left shoulder. Employee was diagnosed with a rotator cuff tear and began missing work on 10/18/2024.
Employee grabbed handle on side of motor to help pull self into the jump seat. EE felt a pop and pain in shoulder. Injury was not the result of a training or compliance issue.
Employee was throwing a track switch and felt pain in shoulder. This employee did not miss work because of the incident until 9/13/2024. Employee was placed off work at that time.
Employee was shoveling a rib roll and felt pain in shoulder. This employee did not miss work because of the incident until 1/29/2025.
Employee was hosing slope and coal was building up around a piece of guarding. EE adjusted the guarding in order to hose around it and it hit the belt and it accidentally sprung back and hit EE's finger causing injury.
Raking concrete inside form, boots stuck in rebar, fell & concrete entered boots.
Employee was carrying a middle roller weighing about 4.5 lbs. Employee slipped and roller came down on EE's left 1st finger.
Employee was traveling to the working section and picked up a container of Blue Wolf all purpose cleaner. While, traveling the liquid leaked off from the nozzle causing a chemical burn to the right buttock.
Employees were offloading steel set beams from the supply car to the scoop. They were handling one of the legs (uprights) and employee accidentally got a finger caught between the end of the upright and the scoop on which it was being placed.
Employee was moving a rock off the ground next to the miner in 9 South Left Mains. EE pinched their left pinky finger between the rock and the rib while moving it. Resulting in a laceration and a fracture.
Employee was working with crew loading track switch parts onto the scoop. Employee was loading the straight lead and it hit a piece of rail which accidentally caused it to move. Employees finger got caught between the rail and other metal track parts.
Employee was pulling a 2-inch discharge line and felt a pain in right shoulder. Employee was diagnosed with a torn labrum in right shoulder that needs surgical repair. Worked all scheduled shifts until being taken off of work on 4/10 for shoulder surgery.
Employee was in the process of spotting structure on the slope. EE was dragging a belt cradle when it accidentally tipped over. EE tried to catch it and got a finger caught between the cradle and the concrete bottom causing injury.
Employee was working on 1 North Section on the Right side of the section. EE was building a stopping at Bk. 35, while walking with block they tripped and fell and the block landed on their right wrist causing a contusion. Please note the employee was off from 02/19/2024 until 02/29/2024 waiting to see specialist.
Employee was changing a v-belt on a pulley attached to the gear box. While turning the pulley, the employee's hand was caught between the pulley and the belt. Injury was not the result of a training or compliance issue.
Employee's calves were exposed to a modest amount of crib mixture while hooking up the "y" to the supply lines. Boots rubbed calves. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
EE was pulling the miner cathead across the power box. As EE was pulling the cathead slipped off the box striking EE in EE's left knee.
Employee was loading skirt boards on a pallet with another employee and felt pain in back.
Employee was climbing off of the headdrive in the G3 longwall. Their feet were not touching the ground, employee reached up to grab mesh to pull themselves and felt a discomfort in their shoulder. Employee worked until their surgery on 3/4/2024.
The employee was fixing some curtain, while putting a pogo behind the curtain they felt pain in their left leg.
The employee was working on #1 North Section and went to get some hydraulic oil. While walking back from oil hole, the employee felt a pop in EE's left shoulder followed by a burning sensation.
Employee was pushing a hanging roll of belt and the lifting device came out of the roll of belt and hit employee in the left side of face.
Employee was controlling DAC phone to stop face chain at #7 shield. A rock was jammed at the headgate while trying to remove rock the rock spun pushing up against removable handrail. Handrail broke free striking employee in the right leg. Employee was diagnosed with a non displaced fracture on 10/30/23.
Employee was carrying 2 pumpable crib bags over their right shoulder and felt a pop and pain in their shoulder. Injury was not the result of a training or compliance issue.
Employee was in 1B Section loading a strap sled when EE's right middle finger was caught and pinched between the sled and the standard. Employee diagnosed with a finger tip fracture and received three stitches.
While installing a 16' cable bolt, drill steel became stuck, employee stepped up on drill head to remove steels. When EE stepped down EE strained EE's left leg.
Employee was helping bolt and was in the process of adding another drill steel when the drill steel in the top accidentally fell out and came in contact with employees wrist causing an injury.
Employee was removing an obstruction from the walkway when employee felt a pain in left shoulder. Employee continued to work all required shifts until being taken off of work to repair a damaged rotator cuff on 11/2/2023.
Employee was lifting a box of bearings with another employee and felt pain in middle of back
Employee was removing a piece of metal from a machine. The last two bolts holding the metal to the machine broke and the metal fell onto the employee's right thumb causing a laceration.
Employee was placing a riser on a catch basin and felt a pop in shoulder
Employees was attempting to change a pin on the slope tail circle conveyor when some debris came in contact with the employee's upper lip causing injury.
Employee was moving a rock out of the way of the miner cable, when the rock rolled onto right ring finger causing a laceration. Employee received 5 stitches to close the laceration. Employee did not miss any work as a result of this incident
Employees were welding plastic pipe when the jaw of the pipe welder broke at the hydraulic cylinder. As the employees were attempting to remove the pipe from the jaws to remove and repair the damaged jaw, the chain holding the jaw up slipped off and struck the employee in the face. The employee recieved a laceration to the nose/cheek area and a broken nose.
On the 1 North Construction site for the 1 West Overcast, while sliding a 6 inch I-Beam into position employee's left hand was caught between I-Beams
Employee was working on a roadway and shoveling concrete material onto the ground. Some dust from the concrete accidentally got into EE's eye causing injury.
