He was carrying a box of resin from back of bolter to front of bolter. He felt something pull in lower back. [He never went to doctor or missed work until 5-21-14].

Employee was lifting belt structure and felt pain in right lower stomach
Employee was hanging miner cable for S-car roadway and when he lifted miner cable above his head got a sharp pain in his abdomen. He worked his reg. job until 1/20/2012 and had surgery for a rupture.
Assistant mine foreman was helping mine foreman load a rock duster and pulled right shoulder. Employee worked his regular job until 1/20/2012 and had surgery on 1/30/2012. Returned to work on 1/24/2012.
Employee was working in 4 entry near face on 14-10AA miner. Putting cat pads together when cat pad slipped out of hand mashing finger between cat pad and pan of machine.

THE EE WAS KNOCKING A HOLE IN A STOPPING FOR VENTILATION PURPOSES IN AN AREA THAT WE HAD MOVED OUT OF. HE WAS USING A CINDER BLOCK TO KNOCK THE HOLE.
THE EE WAS INSTALLING A CONVEYOR CHAIN IN A SHUTTLE CAR. HE WAS PULLING ONT HE CONVEYOR CHAINS WHEN STOPPED TO PUT A LINK UP ON THE CHAIN GUIDES. HE GOT UP UNDER THE SHUTTLE CAR TO BEAT THE LI NK OUT STRAIGHT AND WHEN THE CHAINS CAME LOOSE IT MUST OF HAD PRESSURE ON IT AND THE FLIGHT STRUCK HIM IN THE HEAD.
WHILE PUTTING WATERLINE TOGETHER TWO PCS. CAME TOGETHER AND CUT HIS FINGER.
INJURED WAS CARRYING ROCK DUST TO BELT LINE WHEN HE THINKS HE HIT HIS HEAD ON A ROOF BOLT CAUSING PAIN IN HIS LEFT HIP AND LOWER BACK.
EE WAS DROPPING HIGH VOLTAGE CABLE COUPLER DOWN DURING A POWER MOVE. THE COUPLER WAS HANGING OVER THE POWER CENTER ON 2 WIRES. WHEN EE UNTIED ONE WIRE THE SECOND WIRE BROKE. THE COUPLER FELL D OWN AND SMASHED FINGER ON TOP OF POWER CENTER.
MAN WAS LIFTING 6" HOLLOW BLOCK (CEMENT) AND TWISTED LOWER BACK.
EE WAS LIFTING LEFT LEG UP TO GET IN MANTRIP AND TWISTED LOWER BACK. THIS IS A RE-OCCURANCE OF AN EXISTING BACK INJURY.
MOVING 60 LB RAIL INTO PLACE TO BOLT TOGETHER. HE CAUGHT HIS HAND BETWEEN 2 RAILS.
