Showing 50 of 60EE was holding the end of a grease hose on to a grease fitting when grease shot out and went through EE's glove and into the end of EE's finger. EE received 2 stitches in the finger where the doctor lanced it open to clean the wound.
EE was changing oil on a rock truck and was retrieving a ladder from under the truck. EE picked up the ladder and turned and hit the side of their head on the bumper.
The EE was moving a full piece of 60lb rail to cut a section off of it. When EE slid it to the side it fell down smashing the end of EE's left middle finger.

EE was checking the resistance of a rotor of a drive motor and pinched his fingers between the rotor and Brake caliper fracturing the tips of 2 fingers.

EE had just replaced a drum of grease on his Lube truck and he screwed his grease hose onto it. When he shifted the lever to charge it with air, the end blew off and hit him in the chin. He received stitches.
Injured party was disconnecting a pull rope off a piece of eq. He bent over and was pushing and pulling on the pull rope when he felt a pop in his chest.
Party lifted a sprocket segment which weights about 70lbs out of a box. He felt a pain in his lower back. Missed 1 day of work.
Party was doing a pre-op check on his Cat 777F back dump truck. As he grabbed a hold of a handrail to bend over and look at his truck engine a piece of rust fell behind his eye wear into his right eye. He tried to wash it out but had no luck.
Employee lifted a 3" water pump to set it up to pump water and when he turned he injured his back.
Injury was laying perferated plastic pipe for drainage. The pipes are in 10 foot sections and fit very thightly. He was turning the pipe to make sure the drain holes were lined up when the pipe was shoved together, His right index finger was cut off right behind his finger nail.
Injured was climbing up onto his Front End Loader and didn't see that the engine compartment door had come open and he ran into it causing a cut on his cheek that took stitches.
EE was working on a brake caliper that was locked in his vice and it slipped out and hit his hand. Had 3 stitches put in hand.
On the morning of the 13th EE was climbing down off D11 dozer, when he slipped and hurt his knee. Worked all week on it and on Monday the 20th reported it. Took off Tuesday the 21st and is still off.
On April 10th EE was installing a belly pan on a D11 dozer when the pan shifted and hit his hand. EE thought it was bruised. He went to the doctor on May 25th because he had some pain, they found that he had a hairline fracture on his thumb,they did not put a cast on because it had already started to heal. They reported on May 26th to me.
EE was putting a swing motor into a PC750 Trac Hoe. The gear wedged itself in the mounting hole and when he tried to straighten it up it slipped and pinched his thumb between the gear and mounting bracket. Went and had 5 stitches. Worked the next day.
While helping to install belly chain drive shaft, employee injured his left thumb when a guard swung down and hit his thumb while he had hold of the shaft. He was treated at a Medical facility. Had to have stitches.
EE was moving parts around in his delivery truck to make room for more parts. He smashed his finger between metal lifting bracket and side of bed. Had to go for stitches.
Suction hose on a 4 inch water pump become covered with dirt, causing it to have some pressure on it. When EE unhooked hose from pump, it flew up and hit him in the nose. EE had to have 4 stitches.
EE was putting hose for treatment of water in ditch, when the hose momentarily got caught on pole that holds the hose when not in use. When the end of the hose came loose it shot water into his right eye. Took him to hospital, was told to take 4 days off.
As employee was stepping off bottom step of ladder on his 988 loader he caught his wedding ring on a step. They had to cut the ring off his finger; took him to hospital for seven stitches.
Was picking rock out of coal on conveyor belt,rock rolled over and smashed finger.
Was leaning down under a radiator putting bolts back in the supports, when trying to get back out he pulled on an air hose which in turn pulled air gun down on his head.

EMPLOYEE WAS ATTEMPTING TO PULL A FLEXABLE STEP DOWN THAT WAS BENT UP UNDER THE BUMPER AND WHILE PULLING, THE METAL BASE LACERATED HIS HAND.
Two employees were unloading a pump out of the bed of a utility truck when one employee slipped and the other employee took all of the weight in an awkward position and twisted his back.
EMPLOYEE WAS STANDING ON BENCH BUMPER OF UTILITY TRUCK LIFTING A HYDRAULIC PORT-A-POWER INTO THE BED OF THE TRUCK OVER THE TAILGATE WHEN HE FELT A POP IN HIS LOWER BACK.
During a belt and power move, two employees were setting a feeder by using 4' x 8' headers. While doing this, employing felt a pull in his back.
EMPLOYEE WAS ADJUSTING THE 6 INCH WATER HOSE BY PULLING ON IT AN FELT A PAIN IN HIS LOWER BACK.
EE WAS ATTEMPTING TO REMOVE THE DISCHARGE HOSE FROM A 4 INCH WATER PUMP WHILE THE HOSE WAS STILL UNDER PRESURE. ONCE THE CLAMPS WERE RELEASED, THE METAL END OF THE HOSE SPRUNG BACK AND HIT EE IN THE MOUTH.
Employee was assisting in moving the miner cable and felt a pull in his low back while lacing the miner cable along the rib. Employee continued to work. First day missed was 2/18/05.

EE WAS ATTEMPTING TO REMOVE PART OF A PANEL FROM THE ENGINE COMPARTMENT OF A CATERPILLAR 992C FRONT END LOADER WHEN THE EDGE OF THE SIDE PANEL BUMPED HIS LEFT KNEE.
