Employee placed his right hand in a pinch point area of the roller and boggie pins while trying to install a chain before performing work on the roller.

Employee was cutting on the bed of a 777C Rock Truck and the springy part of the metal broke and threw the employee approx. 2 feet in the air and as employee came down the metal landed on his left fore arm causing a small laceration on employee's forearm needing 8 stitches.

Employee was lifting metal out of the bed of his pick - up truck, as he was shifting his weight to throw the metal from the truck he felt a sharp pain run up his left side.

Employee was taking out top pin of 992C Loader boom, (Boom Pin) while pushing up on the pin employee felt sharp pain in his lower left side below his rib cage, causing him to pull a muscle.

Employee was lifting 45lbs wet hole bag from truck and was placing it into the drill hole (blast hole) Doing so Employees back began to hurt.

EE was pulling a 6" pump hose.

EE was stepping up on the boarding ladder. As he began climbing up on the ladder, he felt a very sharp pain shoot up his lower back. The pain progressed when he started working, causing him to leave early.

Employee was tossing bails of multch on seeder truck and felt a sharp pain shoot up his right side neck.

Employee was assisting in removing the 4" discharge hose which was wrapped around pump during transport. As employee pulled on hose, which had become tangled, the coupling on end of hose came loose and struck employee.

Employee was walking up the slope and dropped his water bottle. As he bent over to pick the water bottle up, a stick from the trees he was carrying went up his nose, puncturing the skin inside his nose causing it to bleed.

WAS CHANGING OIL SCREEN ON 992 LOADER WHEN A PIECE OF METAL FELL IN LEFT EYE.

WHILE CHECKING OIL IN LOADER, EMPLOYEE SQUATED DOWN; LEG AND ANKLE POPPED.

EE WAS MOVING WOODEN BLOCKS & ALSO PULLING WATER HOSE, WHEN NOTICED PAIN IN STOMACH.

WHILE PLACING A PIPE ACROSS POND DAM; CHAIN SLIPPED OFF OF PIPE, PIPE HIT EE ON OUTSIDE OF L. KNEE.

AFTER SHOT WAS PUT OFF DRILL MAN MANUALLY LIFTED AND MOVED ROCK APPROX. 15 FT. AFTERWORD LOWER BACK AND GROIN BEGAN HURTING. CONTINUED TO GET WORSE BUT EE WORKED THURSDAY AND FRIDAY. UNABLE TO WORK MONDAY 1-29-01.

WHILE REMOVING A LINE ON A DOZER HAND WAS PRESSED INTO ANOTHER LINE, REQUIRED 4 STITCHES.

AFTER REMOVING SEAT FROM TD25G EE LIFTED AND TURNED AROUND PULLING A MUSCLE IN BACK.
