EE WAS REMOVING A HOSE REEL FROM THE REAR OF THE SERVICE TRUCK, SPRAYED THE BOLT WITH 6 IN 1 LUBRICATING OIL TO LOOSEN THE BOLT. HE WAS PULLING ON THE BOLT HE FELT A TWINGE IN HIS LOWER RIGHT ABDOMINAL AREA. HE DID NOT HURT VERY MUCH & CONTINUED TO WORK, LATER WHILE EATING LUNCH IT WAS UNCOMFORTABLE TO SIT DOWN,M AND WASN'T FEELING WELL. HE CONTACTED THE LEADMAN AND WAS TAKEN TO TH
EE SAID HIS BACK WAS SORE BETWEEN HIS SHOULDER BLADES AFTER MOVING CABLE FOR ABOUT AN HOUR. HE ALSO SAID HE HAD SOME NUMBNESS IN HIS RIGHT ARM. THIS WAS DIAGNOSED AS A CHRONIC DEGENERATIVE CON DITION.
WHILE EE WAS STANDING ON BLADE CLEANING WINDOWS, WHEN HE REACHED TO CLEAN THE BACK WINDOW HE FELT A TWINGE INHIS LEFT HIP.
EE REPORTED THAT HE BUMPED HIS HEAD, JAMMING HIS NECK WHILE CLEANING THE DRAGLINE TUB. AFTER SEVERAL WEEKS, HE WAS PRESCRIBED FOUR WEEKS PHYSICAL THERAPY, PRESCRIPTION DATED 9/6/01.
EE SAID HE WAS CHANGING A GREASE REEL ONT HE REAR OF A SERVICE TRUCK. THE REEL GOT HUNG UP, AND WHEN HE REACHED AROUND TO FREE UP A LINE, THE REEL SLID OFF AND LANDED ON THE INSTEP OF HIS RIGH T FOOT.
EE HAS REPORTED NUMEROUS INCIDENTS RELATING TO SHOULDER PAIN. A VISIT TO AN ORTHEPEDIC SURGEON RESULTED IN REQUEST FOR MRI
EE SAID HIS LEFT KNEE SWELLED UP AFTER HE HAD BEEN KNEELING ON A GRATE TO WELD A HANDRAIL. DIAGNOSIS AND TREATMENT HAVE BEEN FOR BURSITIS.
