Employee removing hydraulic pump from tire truck when it slipped and pinched a finger. X-rays subsequently revealed a non-displaced terminal tuft fracture.

Employee reported when moving a rock knocker that had fallen off a haul truck, EE felt a pop and pain in EE's left arm. Surgery performed 3/19 for a left elbow distal tendon rupture repair.

While seated, co-worker patted employee's knee. The employee reacted by reaching to push the co-worker's hand away. Hands made contact and a loud "pop" occurred. X-rays revealed fracture of the 4th metacarpal.

Employee reported while positioning a heavy metal stand, the stand toppled injuring two fingers on the employee's left hand. The injuries required medical treatment and resulted in time off work.

Employee was assisting with a cable change on a boom winch. His left hand was pinched when the weight of the cable shifted while getting the cable back on the sheave. This injury resulted in a laceration requiring sutures.

EE investigating oil leak under 777 haul truck during pre-op inspection on March 18. Reached up with rag to wipe some dirt from hose. His light went out. Dirt fell on his face behind safety glasses. Blinking, couldn't see, stood up and hit his head under the truck. May 9 employer was informed of a bulging disc from this incident. Work restriction imposed May 6. No missed work days.

Employee pinched his left hand ring finger in the door stop on the rear door of the dragline. This injury resulted in a laceration requiring sutures. The employee did not miss any work time or was restricted as a result of the injury

Employee reported while reaching over to close the door on the loader, he felt pain in his lower back. Pain developed into severe muscle spasm requiring medical evaluation. He was restricted from full work duties as a result of this injury.

When leaning forward to grab the ladder, the ladder swung back pinching his left index finger causing a laceration. Employee received treatment in the form of sutures. Employee was removing a tire and rim assembly from a trailer when he felt a pop on the top of his right shoulder. MRI completed 4/14/08 showed severe degenerative changes in the AC joint requiring surgery.

EE working under pickup when debris fell into right eye. EE treated by a Physician who noted foreign body and rustering in the right corneal rim. Antibiotic ointment was instilled and will be administered for the next several days by EE. He reported injury on 1/22, doctor notes received on 1/30. He has not missed any work time or been restricted as a result of this injury.

While lifting dragline power cable, employee pulled something in his lower back. Past medical history of chronic lower back pain and has had previous back surgery. The employee was released to full work duty without restrictions upon initial doctor's visit. Subsequent appointment resulted in a lifting restriction. Employer was notified of lifting restriction on 12/6/04.
