The employee was lifting a steal cover off of a roof bolting machine. As he lifted the cover he twisted his back. He went to the Dr. on 2/23/2012 and was put on restricted duty.

The employee was hanging insulated wire. The wire flipped over and hit him in the eye.

The employee was loading a reducer for a Joy Battery Ram car. He slipped as he picked up the reducer and it fell on his thumb.

The employee was moving belt structure. As a co-worker moved a bottom roller it hit his finger. An accident report was filed that day. He was seen by a Dr. on 10/12/11 due to still having pain in his finger. Medication was prescribed to aid in reducing the swelling. The employee did not miss work and was not placed on restricted duty.

The employee was advancing a belt line. He hit his finger between a bottom roller and the coal rib. No fractures were found but a prescription was given for pain.

Employee was stopped on equipment. He was adjusting his CB radio antenna, which is located on the outside of the equipment. While moving the magnet to which the antenna is attached, the antenna flipped back and hit him in the right eye. The sclera (white part) of his right eye was lacerated.

The employee was moving a stored SCSR Cache and felt a tear feeling in his abdomen area. After consulting with a doctor on the following Monday it was determined that the employee was in need of surgery do to a hernia. The employee was placed on restricted duty.

The employee was loading bottom rollers onto a structure trailer. As another employee threw a bottom roller it caught the injured employees finger between two rollers.

The employee was retrieving HV cable. As he and three other miners were pulling on the cable it snagged on a crib. As the cable suddenly stopped the employee felt a strain in his groin and stomach. After consulting a DR. on 2/25/11 it was determined he has a hernia and will need surgery.

The employee was changing a hydraulic hose on a continuous miner. The head pin plate was in an upright position to allow access to the damaged hose. The head pin plate fell on the employee's hand.

The employee was installing roof support. As he was putting his pusher steel into the starter steel it snapped back striking his thumb.

The EE was attempting to remove drill steel that had become stuck in the mine roof. He pulled downward on the drill steel, with both hands, at eye level. This action caused a strain of the lower back muscles. The EE missed one work day and has remained on light duty since.

The employee was hanging miner cable using plastic coated tie-wire. An end of the wire struck the employee's right eyeball causing an abrasion of the eyeball. The employee was wearing safety glasses at the time of the incident.

The employee was assisting in the removal of a wheel unit from a battery coal hauler. The unit was suspended on the forks of a mini-trak by a chain. The wheel unit rotated striking the employee's right knee. The accident occurred on 1-8-08. The EE did not have trouble with the injury until 1-13-08. He was seen by Dr. on 1-14-08. Same knee operated on in 2003, different reason.

The employee was hanging PVC discharge pipe in the face area of the # 3 entry on MMU-002. He reached up for the pipe and a piece of steel tie wire punctured the back of his right hand. He was sent to the hospital for cleaning of the wound and a tetanus booster.

The EE was building an intake solid concrete block stopping. The wall was over 8' high. The wall was not properly capped for the wall's size. The wall fell, striking the EE. The EE sustained a laceration of the scalp and minor bruising.

The EE attempted to pull up his miner's belt on his hips prior to kneeling to clean dustbox on roof bolter. Unk to EE belt knife had opened in tool pouch. Index finger contacted exposed blade causing a laceration. EE taken to ER for cleaning & stitches.

The employee was moving a roof bolt. A burr existed on the side of the bolt. The burr grazed his forearm causing a small puncture wound. The injury was not reported because the employee felt it was insignificant. Two days later the wound became infected causing him to be hospitalized for treatment.

The employee was shoveling the unit tail piece. He lifted a shovel full of coal from the mine floor to put it on the belt. He twisted wrong causing a strain of the lower back.

The EE was installing a top-roller into position during a belt move. He was lifting the roller plus the weight of the conveyor belt. His position in relation to the weight being lifted caused a strain to a muscle in his lower back.

The employee was attempting to direct the flow of concrete during a free-flow pour. The concrete level rose higher than his boot tops. This caused burning of the skin around both calves. The concrete also contacted his skin directly above the knees on both legs causing further burning of the skin. No medical attention needed.

The ee was dragging steel arch pieces for assembly. The arch piece was dopped smashing the tip of his left middle finger. He was sent to the hospital for x rays. x rays were negative for breaks. Stitches were put in. He returned to work 2 hours later.

The EE was loading timbers onto the roof of a mantrip. The timber was dropped, trapping the tip of the EE's finger between the timber and the mantrip roof. X-rays showed a slight fracture.

Employee was lifting an arch section off the top of a mantrip. The arch became stuck on a bolt head protruding from mantrip. Steel beam dropped severing the tip of finger. He was taken to the hospital for x-rays. No broken bones were found. He lost skin only in the accident.
