INJURE DSTATED THAT HE WAS IN #2 ENTRY SUPPLYING THE ROOF BOLT MACHINE AND HAD LOADED SIX BOXES OF RESIN BLUE ONTO THE ROOF BOLT MACHINE WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK ON THE RIGH T SIDE. RESUTLED IN STRAIN TO LOWER BACK.

EE STATED THAT HE WAS WORKING ON JOY MINER IN 4 ENTRY. HE WAS CHANGING FILTER UNDER LID ON OFF SIDE OF THE MINER. I HAD THE LID IN MY LEFT HAND AND TURNED TO GET A WRENCH AND THE LID FELL ON M Y RIGHT MIDDLE FINGER. CAUSING LACERATION AND FRACTURE CHIPPED BONE TO RIGHT MIDDLE FINGER. LACERATION DID NOT REQUIRE SUTURES.
