While moving a yield sign on a skid, the employee's right pinky finger became trapped between the skid and the ground. The employee jerked his finger free and in doing so caused a fracture to the tip of the finger and a laceration that took eleven sutures to close. After treatment, the employee was given a full duty release from the ER doctor.

Employee was laying down some metal shelving on a push cart when his left ring and pinky finger were caught in a pinch point causing the ring finger to be lacerated and the pinky finger to lacerated and broken. Employee lost approximately 25% of the distal bone of the pinky.

Employee was closing the door on the BE 1300W dragline that enters the PCL room when his glove caught on a metal burr on the door latch which caused the employees left index finger to be smashed in the door. This resulted in pinching off of the flesh of the tip of the finger.

Employee was lining up a lift cylinder to a D-block on a D-11 Cat dozer. During lining up of the cylinder and the D-block. The employee's left hand was caught between the D-block and the gusset on the dozer blade resulting in fractures to the left middle and ring finger.

Employee was removing bolts from the belly pan of a scraper to perform maintenance work when the hinged pan swung down pinching his right index finger causing it to be lacerated on both side from the middle joint up. This resulted in 17 stitches.

After securing the rope on a cable tree rope brake, the employee pulled sharply on the free end of the rope which caused the rope brake to swivel towards the employee. The handle on the rope brake struck the employee on the right eye causing ocular fractures. The employee was wearing safety glasses at the time of the incident.

A rubber tire dozer operator was exiting the dozer cab at the end of the shift. When he shut the cab door, his left ring finger tip was pinched between the cab door and the door frame. The operator sustained fractures of the finger tip and a laceration of the finger tip, requiring six sutures to close.

EE was operating a locomotive door with no safety features to prevent inadvertant closure. The EE was holding a clip board and lost controll of the door as his hand attempted to clear the door. The door closed on his left ring finger post to the begining of the nail bed, causing a laceration and parial oblique amputation. No loss to use of finger.

Employee was climbing down a ladder on a loader and lacerated his right middle and ring finger on a burr. He needed sutures to close the wound. He returned to work on his next regular scheduled shift with no restrictions.

Employee was removing a cullet from a pin. Employee lifted the pin off the ground using a remote control and shop crane (wearing no gloves). Two other workers were using a 8# metal hammer to "tap" cullet free from the pin. Injured employee moved 8' away from work area to lay the remote down. A metal sliver came and imbeded in employees top side right hand.

EE was lifting a bucket of approx. 20 to 25 lbs. of bolts and other materials from the ground to the flatbed of his pickup when he sustained an umbilical hernia. He was evaulated by a physician and fully released. On 2/21/08 the employee underwent outpatient surgery to repair the hernia and received a full work release to return on his next regularly scheduled shift.

Employee was closing the door to the 16g motorgrader and accidently closed it on her right middle finger that was in the door jam. She fractured the tip of her middle finger and received 8 sutures to the laceration. She returned to work on her next regularly scheduled shift.

EE was placing a chock block back into holder on Haul Truck #2043 when a piece of mud fell off underside of the truck and struck her. She was given a full duty work release in the emergency room & returned to work. One week later she went back to doctor on 9/7/07 and was given restricted duty until MRI results come back.

While laying a dozer blade pin down, the pin shifted in the cardboard box containing the pin. This caused the pin to slide out of the box pinching an employee's finger causing a laceration to the end of the finger. In addition, the employee experienced a tuft fracture to the end of the finger. The employee was wearing gloves at the time of the accident.

EE was attempting to place the de-watering pump back into the cradle after he was finished de-watering a blasthole when pump slid into postion pinching the tip of the his finger between the cradle and the pump. He was in compliance company and mine rules and regulations.The accident did not involve the mining system, job skills or protective equipment.

Certified electrician was turning a rotor on a haul truck wheel motor to start profiling. As he pulled down on the rotor he heard a "pop" sound in his right arm and immediately experienced pain in the area. X-rays revealed he had ruptured the distal tendon in his right upper arm.

Employee caught left index finger between a pin and dipper bail support arm after the bail moved unexpectedly. He received a 1/2 inch laceration to the tip of his finger which required 7 sutures.

Employee was assisting an oiler take an oil sample on an 854 Catapillar RT dozer. The oiler asked him to shut the machine off and lock it out. When he returned, he stepped into the engine compartment and lacerated his left lower leg. He was transported to the hospital and received 6 sutures to close the wound. He returned to work on his next scheduled shift.

While trying to remove a wire choker from a haul truck the employee's left middle finger was lacerated by a sharp broken wire strand which cut thru the leather glove. The injury required stitches.

A scaffold collapsed while being dismantled. A welder on the lowest level fell about four feet to the ground, sustaining a lower leg fracture and damage to the ligaments in the right ankle.

The employee was removing an inspection plate of a D-11 dozer to change transmission fluid. The plate would not come off so the employee was going to get a pry bar. He placed his hand on the floor to scoot out from under the machine when the plate fell from the dozer onto his right hand.

Employee was positioned inside of a manlift installing a pivot arm cap on a tailgate pivot arm of an 830E Komatso haul truck. The employee placed two bolts in the cap and was reaching down to pick up another bolt when the cap came loose stiking the little finger on the right hand which was positioned on the handrail of the manlift.

EMPLOYEE WAS PERFORMING A PRE-SHIFT WALK AROUND INSPECTION ON HAUL TRUCK. WHEN SHE LOOKED UP TO INSPECT THE TRUCK FRAME SOMETHING ENTERED HER RIGHT EYE BEHIND HER GLASSES. SHE WAS TAKEN TO THE HOSPITAL AND DIAGNOSED WITH DIRT IN HER EYE. IT WAS SWABBED OUT AND SHE WAS GIVEN A PRESCRIPTION ANTIBIOTIC TO PREVENT INFECTION. SHE WAS RELEASED TO FULL DUTY THAT DAY.

While lifting trash bag out of trash can, employee also lifted the trash can. The trash can fell on her toe. At the time, employee did not seek medical attention. She went to the Dr. on July 21st and had the toe nail removed.

While lifting a sample the technician said she injured her elbow. This was diagnosed as tennis elbow and became reportable 3/15/05 due to lifting restrictions given during a follow up appointment. Workers Compensation has been filed, but company is contesting the case and this report will be reviewed for further action pending outcome of the appeal.

EE WAS INVOLVED IN INSTALLING A SET OF DUMP ROPES ON A DRAGLINE BUCKET. AS THE NEW ROPES WERE BEING REMOVED FROM THE REEL ONE SET OF ROPE ENDS RELEASED TOWARD THE EE UNEXPECTEDLY. HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN THE ROPE END & SERVICE TRUCK. COMPOUND FRACTURE TO THE LEFT INDEX FINGER.

THE EE WAS IN THE PROCESS OF MOVING THE BELT SPLICE MAGNET. HE SET THE CONTROLLER DOWN & WAS RUNNING THE CONTROL BUTTONS AS WAS TRAMMING THE MAGNET. THE DOWN BUTTON STUCK & LOWERED THE MAGNET ONTO HIS MIDDLE FINGER WHICH WAS STILL ON THE CONTROLLER. THE EE PULLED HIS HAND OUT FROM UNDERNEATH THE MAGNET CAUSING THE AVULSION TO THE RIGHT MIDDLE FINGER.

WHEN HE THREW THE BLOCK UNDER THE FRONT OF THE ENGINE THE BLOCK CAUGHT ON THE GLOVE HE WAS WEARING AND PULLED HIS FINGER UNDER THE ENGINE WHERE IT WAS PINCHED BETWEEN THE BLOCK AND THE ENGINE.

THE EE REIEVED FOUR SUTURES.
