Employee sustained injuries while trying to secure a miner cable into a multi-hook hanger that was beyond EE's reach. The employee attempted to jump to place the cable into the hanger but missed. EE's hand became caught between the miner cable and an adjacent roller hanger, leading to a laceration and a metacarpal fracture of EE's right hand.

A Mechanic removed a fender from a bulk truck and with help lifted and placed the fender on the ground. After setting fender down mechanic felt pain in EE's back and sat down in hopes the pain would pass. Employee was transported to hospital and diagnosed with lumbar spasms and required to miss one day of work to rest.

Employee was removing a Kennedy panel from a stopping. As EE looked up to see what the top of the panel was caught on, a baseball size rock with sharp edges fell from the top striking EE in the face and lacerating EE's upper lip.

Employee was operating a skid steer stemming holes on pattern. Once finished, the employee parked skid steer and went the lift the lap bar and exit skid steer. Employee received 1st Aid for pain in the right shoulder and returned to work. On 10/15/2024 , employees rotator cuff was repaired and was placed on leave by the medical provider.

While dismantling the monorail on the longwall tailpiece, the monorail slipped from the injured person's left hand and struck the tip of their right-hand pinky finger. This resulted in a crushing injury to the fingertip and the fingernail, necessitating six stitches to close the wound. The injured individual was released without any restrictions.

Note: The original date of injury was 8/6. The employee continued to work until November 22 at which time the results of an MRI revealed that time off work would be needed. Description: Employee smacked the drill steel against another piece of steel while rib bolting to get it unstopped. EE felt a sharp pain radiate from EE's shoulder up into EE's neck.

An employee was making adjustments to a shovel bucket, when the stand kicked out from under the latch bar and it pinched the web of the employee's thumb, causing a laceration that required stitches.

Employee was getting a chain into position around body retaining pin to aid in pulling a lower strut pin. While holding the body retaining pin up placing chain into position, the pin slipped out of their hand. When pin fell it pinched their middle and ring finger of left hand between body retaining pin and differential housing.

EMPLOYEE WAS ATTEMPTING TO CLEAN A CLOGGED SPRAY NOZZLE AND WAS SPLASHED IN THE EYE BY WATER AND DIRT. EMPLOYEES EYE WAS FLUSHED AND TAKEN TO HOSPITAL WHERE IT WAS CLEANED OUT BETTER AND EE WAS GIVEN A PRESCRIPTION MEDICATED EYE OINTMENT.

ANOTHER EE HEARD PANEL SLIDE ACROSS OTHERS, LOOKED AROUND STOPPING AND SAW INJURED WORKER ON THE GROUND. EE RETURNED TO NEXT SCHEDULED SHIFT, THEN SAW MD 3/13/24 FOR FOLLOW UP AND WAS PLACED OFF WORK FOR SYMPTOMS OF POSSIBLE HEAD INJURY.

While assisting in loading continuous miner cable into the boom of the machine, via the use of a scoop, using a steel sheave wheel and nylon cable. The employee placed their left hand onto the cable and pulled fingers into the sheave wheel, resulting in injury to three of the fingers on left hand.

Employee was installing the fork back onto the forklift. While trying to get it into place EE's left-middle finger was caught between the back-clip of the fork and the cleat of the fork-frame. EE received 2 stitches in the left middle finger.

While bolting, the employee was placing a piece of mesh on top of the ATRS and adjusting it with EE's hands. A rock fell and caught the inby corner of the mesh and caused the mesh to spin around and strike EE in the mouth area and knock out 2 teeth.

Employee was pumping grease into the idler pump on a continuous miner to insert a shim. While inserting the shim, the grease hose fitting broke causing the hose to become detached. The employee was struck in the side of the face by the pressure released from the fitting.

Individual was injured trying to move a piece of coal off the back board that measured 56 inches long by 28" wide which struck EE once it hit the chain. This caused it to flip up hitting EE's face causing EE to fall backwards hitting the back of head on the shield.

Two employees were hanging bottom roller on structure during a belt move. The roller was being lifted simultaneously by each employee with the bottom belt laying loose over top. The injured employee felt a pop in shoulder. Note: The employee worked until 6/12 when employee was then required to have surgery to repair damage sustained during this action.

While attempting to change the batteries, personnel were trying to lift the new batteries with the scoop bucket and place cribs under the batteries. Injured placed their hand under the batteries to remove debris when the batteries slid on the metal scoop bucket. Injured was diagnosed to have a fracture of the second knuckle on the right ring finger.

THE ASSEMBLY HAD BEEN STOOD UP AND CREW WAS GETTING READY TO REPOSITION ROPES TO MOVE ASSEMBLY FINAL POSITION. EES WERE BELIVED TO HAVE WENT IN TO MOVE ROPES WHEN THE ASSEMBLY FELL OVER STRIKING THEM.

THE ASSEMBLY HAD BEEN STOOD UP AND THE CREW WAS GETTING READY TO REPOSITION ROPES TO MOVE ASSEMBLY INTO FINAL POSITION. EE IS BELIEVED TO HAVE WENT IN TO MOVE ROPES WHEN THE ASSEMBLY FELL OVER PINNING THEM.

Driver started the belt on live bottom trailer forcing load against tailgate before unlatching tailgate. EE tried to free tailgate by hitting latch with a chain hook. This caused the tailgate to break free and hit the driver in the mouth.

The employees were pulling in/out 1000' of belt. While the injured employee was pulling the redwing (AKA Duck-Bill) over by hand to keep the belt straight the redwing slipped off while under pressure and struck the employee in the forearm.

Employee was unspooling and feeding electrical cable down the shaft of the time. The spool was freely spinning on a 1" piece of rigid conduit that was supported on racketeers. The momentum and weight of cable caused spool to spin uncontrollably. The spool and conduit struck the employees. Approx 16 stitches.

Employee was installing a 10ft. cable bolt when it became stuck in the drill hole. The employee was attempting to shove the bolt up manually whenever employee felt discomfort in right shoulder. The employee started missing work on 5-3-22.

Employee was turning motor sheave on unit 214 on the 3rd floor of the prep plant to assess the condition of the motor. When the employee rotated the belt, employee's right thumb was caught between the belt and the sheave. This resulted in a laceration to the right thumb that required five sutures.

There were 2-3 rows of block left attached to mine roof that became dislodged as the witness was straightening blocks already stacked in place. A block fell causing a fracture to the injured EE's left pinky finger.

While attempting to uncouple the power supply battery cable plug to the scoop; melted insulation from the coupling was expelled, striking the employee on the right forearm. The employee was taken to hospital then transferred to a burn clinic where they were treated for a second degree burn to the right forearm. The employee was admitted overnight.
