EE was lifting a valve into position from under a truck. The valve slipped and when they caught it, they felt pain in their shoulder.
EE was manually moving a barrel of oil onto the containment when it started to tip. EE tried to stop the barrel from tipping and got hand caught between the barrel and a steel I-beam.
Driller and helper were adjusting the height of the control panel while the driller was getting ready to insert the pin the panel was released and smashed EE's thumb causing a crushed fingernail that required 4 stitches.
EE was re-installing the guard onto the tail pulley of BC-5 when the guard slipped from their hands and pinched 2 fingers on right hand.

EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.
EE was loading bolts in the carousel of a rock bolter. EE received a laceration to the left middle finger when it was pinched between two bolt plates.

EE injured back when EE attempted to lift a 6" x 8' piece of thin wall HDPE pipe.
A helper on an underground core drill was lifting a 5' HQ drill rod to add to the drill string when they lost footing and fell to the wooden deck (floor). The drill rod fell with them and landed on right thumb causing a fracture to the thumb.
An employee was loading a mini transformer into the back of a vehicle, when employee got it about half way up the employee's shoulder made a popping sound. The employee did not seek medical treatment at that time. The employee's shoulder did not get better so employee was evaluated on 06/17/2021 by a physician at which time employee was put on restricted duty.
Employee was unloading 4'split set bolts in a laydown. As EE was stacking them, one of the bundles slipped and when EE tried to catch it, EE's left hand ring finger got caught under the bundle of bolts and smashed the end of it.
Employee was attempting to attach the boom on a bolter and a nylon strap EE was using broke and the strap struck the employee in the face.
An employee was helping load a trench round in 15-E, EE was bent over loading a hole in front of the vent bag and the EE next to EE was filling a hole full of stemming material, while doing so some of the dust from the dry material blew in to EE's face getting some into EE's right eye.
Employee experienced discomfort when hanging power cable on the rib. Employee received an MRI on 8/12/20 and was referred for physical therapy for rotator cuff tendonitis making this a reportable illness.
Employee felt a sharp pain in the right side of their neck and right shoulder while pushing a 2200# Tote. The employee did not seek medical attention until December 10th after reporting discomfort while trying to sleep. The examining physician suspects a strained rotator cuff and has restricted work to a 20lb weight limit and no over head lifting.

While removing a vehicle wheel from a stack of 4, the wheel slid and pinched the employee's right index finger.
Employee had removed a jackleg from the front basket of a tractor to get 2 straps. When employee was putting the jackleg back in the basket, the jackleg rolled and cut employee's right ring finger in the chuck of the hammer.
EE was getting a new utility knife and a new razor blade. EE had both the blade and the knife case in their right hand turned and bumped their hand on the wall. The blade shifted and cut their right index finger.
