While cleaning the floor in the SAG mil, employee attempted to throw a 5-inch grinding ball into an elevated ball bin. The ball came back down & struck employee's left middle finger as the were holding onto the lower part of the structure. Impact caught the finger between the grinding ball & the structure resulting in a laceration requiring stitches and a fracture of the finger.

Employee was doing a brake inspection of a Trailer when EE was measuring the slack adjuster a small chunk of dirt fell between EE's safety glasses and EE's eye causing scratches on EE's eye. we took EE to urgent care where the prescribed EE antibiotics.

EMPLOYEE WAS WIPING DOWN HUB SURFACE OF THE LEFT FRONT BRAKE ON 13060. ANOTHER EMPLYOEE WAS REMOVING DUO-CONE SEAL AT THE SAME TIME. THE DAMAGED METAL RING SPUNG FORWARD AND HIT THE EMPLOYEE IN THE RIGHT THUMB KNUCKLE CUTTING IT OPEN, RESULTING IN STITCHES.

Employee was picking up a chock block that was froze down and felt discomfort to their left shoulder. They continued to work and did not want to be seen by a doctor. On April 18th they reported that they were feeling a lot of pain and would like to be seen by a physical therapist. They then received dry needling.

Employee was removing an air starter motor from a 785 CAT haul truck, when oil started pouring out of the starter mount. Employee quickly leaned to the left, while still holding the air starter, and smashed left ring finger between the starter motor and the truck.

Employee was lifting rubber mill liner segments off forklift & placing on liner machine to install inside the mill. Employee felt EE's forearm pop. Employee notified supervisor & was taken to ER. Employee was released to return to work w/further exams scheduled later. MRI results showed torn bicep requiring surgery. Employee missed first scheduled shift on 6/1/23

Mill Maintenance Mechanic was rebuilding a 12" Knife Valve. While re-aligning the valve plate, the piston slipped down and impacted EE's left thumb causing a laceration that required 4 stitches.

A mechanic disconnected a hydraulic hose from the final drive on a CAT 785 haul truck and placed one end up on the frame. As EE was putting plastic in place to cover the final drive, EE bumped the hose. It came off the frame and struck EE in the mouth. The impact from the end of the hydraulic hose cracked EE's upper right canine tooth.

While repairing A/C on a 777 CAT Water Truck a mechanic reached down between seats to check air flow, as EE was reaching down EE stumbled and EE's fingers entered the A/C intake and contacted the fan. Mechanic was taken to ER where finger tip was stitched up.

Employee strained their left wrist and forearm back on 08/08/2018, pushing on the door handle of a 785 CAT haul truck. Surgery had to be performed on 04/21/2021 to remove some scar tissue from the wrist.

On 12/26/19 warehouse-person picked up a box and was putting it on a cart and felt a pain in back. This was reported to supervisor 12/30/19 because pain continued. Upon reporting employee was taken to local clinic. Diagnosis was a back strain & restricted for 2 weeks to lifting <10 lbs. Employee was placed on restricted duty.

Employee was shoveling frozen mud from the track frame of a D-10 dozer when EE felt a strain in right shoulder. Employee went to the doctor for observation two days later. EE was referred to an orthopedic surgeon. This injury went LTA on March 14, 2019.

Employee was removing a Lock Ring from a 785D wheel assembly. The Lock Ring began to fall off the wheel assembly. The employee tried to stop the Lock Ring from falling resulting in the employee getting thumb smashed.

Employee was removing an 8' section of 8' poly pipe from an elevated position on the reject stacker at the SAG mill. The section of pipe dropped a short distance while EE was trying to support it with the arm overextending EE's right arm tearing/detaching the bicep tendon. Results of 6/7/18 MRI revealed the surgery would be necessary.

Employee was changing bit on blast hole drill. When EE reached to straighten the bit to connect it to the drill steel EE hit right little finger on a sharp edge of the breakout wrench. The sharp edge cut through the glove and lacerated EE's finger. The laceration required 4 stitches.

Employee needed to reposition the bit basket in the drill deck. Employee bent over and grabbed the bit basket with the left hand and pulled up on the bit basket. Employee felt a pop and an "electrical shock" sensation go up the left forearm. Injury to tendon in left forearm.

Employee was lowering a piston into a ram of a track adjuster for a D 10 dozer. When piston was in place employee grabbed it and was lowering the overhead crane to unhook lifting straps. The crane hook came in contact with left pointer finger, requiring stitches.

Employee was assisting a mechanic in moving final planetary out of a press. Employee held the hook on the lifting sling while the mechanic took up the slack in the sling with overhead crane. When the slack came out of sling employee's left index finger was caught between the hook assembly & planetary housing. This resulted in lacerating of left index finger requiring 5 stitches

Employee was working in the SAG Mill changing lifters and liners. Employee was lifting a Sag Mill lifter and felt sharp pain in EE's lower back. Employee was taken to receive medical care/evaluation 01/14/2016. Received notice 01/20/2016 that employee is unable to return to work at this time.

Employee was helping install Hilfickers around the inlet and outlet of a fifteen foot culvert. Employee stepped over the four foot rise to retrieve a rake that was stored inside the culvert. Employee lost footing and slipped puncturing upper leg with a 3/8 inch wire located on top of the hilfickers requiring two to three stitches.

Employee was helping replace a section of 14" diameter by 55 long Drisco pipe that ran out through a wall and down into a surge tank. One bolt had been installed in the flange. The pipe was being rotated to make alignment of the holes. The pipe turned and rolled around and pinched employee's right middle finger between a 2" steel water line and the Drisco pipe.

Employee needed to repair a broken wire on a D-11 dozer that was located under cab floor. As employee was in this area he pushed against a hydraulic line, this caused dirt to fall from the hydraulic line onto his forehead and down on the inside of his safety glasses. Some foreign material got into the employee's right eye.

Employee, an SOS contract employee working at Simplot Phosphates, was raising the feed pipe. She put her left hand on the block of the crane to steady it. As the crane block was coming up through the chute it pinched employee's left ring finger between the feed chute and the crane block.

Employee was pulling on an old spool of three inch fuel hose, did not see the wire reinforcement sticking out the end of rubber. He was moving hand to get better grip and wire caught middle finger on right hand. Employee received 6 stitches.

Working with another EE on pump. EE placed a bar to hold sheave and the other EE asked him to move the bar so he could check the threads on the shaft. When the other EE tried to screw on the impeller the EE had not replaced the bar and tried to hold the sheave by the belts. EE's fingers were caught between the drive belt and sheave.

Employee was cleaning work area at close of shift, was using a pressure reducer on an air hose to clean up work bench. There was an accumulation of dust and metal shavings that was hidden between items on the bench; employee had a foreign object taken out of left eye as a result.

Dark, windy night. Employee was washing sand from dryer pad with pressure hose. Hose became kinked and the employee tried to move hose around to get the kink out. Hose was full of pressure and he lost control. The nozzle portion came up and struck employee in the face.

Employee was opening a can of welding rods by pulling on the pop top tab, it hung up and employee grabed the can to get a better hold. Employee was not wearing gloves and caught right index finger on edge of lid, cutting index finger. Required four stiches.

Employee was working in the B pit area on the date of incident and around 2:00 PM. He started to have some irritation in left eye, he filled out an accident packet at end of shift. The next morning he was taken to the hospital and a small foreign body was taken out of eye and antibiotic was prescribed in eye drop form.

Originally believed to be off site injury and became reportable upon acceptance of workers comp insurance. Became reportable 5-19-08 as per notification form workers comp insurance 09-03-08. Accommodations have been made for permanent lifting restrictions.

Employee was running a loader, employee stepped off on to a small rock. He got back on the loader for 30 minutes. Later he complained of knee pain. He was taken to the doctor. Doctor found no damage but there was arthritis and bone spurs. Employee has a history of previous work and non-work related knee injuries. Workers compensation is investigating this incident.

Employee was rotating a drive pump to get the timing right, the pump moved unexpectedly and smashed his hand between the flange and pump. Employee received a cut in the palm of hand and a bruise on the top of hand resulting in three stitches

Mechanic was changing out a welding gas bottle when he felt a sharp pain in lower left groin. He went to the doctor for diagnosis and returned to work until 2/6/2007 when he went in to have a small hernia repaired. He reported this to his supervisor on 2/20/2007.

While removing caustic line that goes to the seal tank some caustic got into his boot. While removing protective equipment EE failed to wash his gloves before removing his boots or suit & goggles. Caused chemical burn on right foot. EE told supervisor @ 8:00PM that this happened. Super visor told him to remove boot.EE told supervisor not a problem only small amount.

EE was running MgO and soluble iron assays on mill samples in the lab at the AA machine when a sample bottle he was holding in his left hand slipped loose. As he attempted to catch it, the sample and bottle struck the lab bench's counter-top, broke and cut emplooyee's lower left hand.

Employee was lifting the chock block into the holder when something popped in the left shoulder. Felt some numbness in left hand. Employee went to a doctor during days off. Doctor did not see anything. Employee continued to work for about a month. Then went in for exploratory surgery on 3/13/06.

The employee was loading a pump impeller in to a bucket of a skid steer loader. The impeller tipped over and pinched employee's finger between impeller and I-Beam.

Employee was removing a small air conditioning unit from an office window. It was frozen to the frame. When the unit broke loose it slipped out the window. Employee grabbed the cord to catch it from falling. The cord wrapped around employee's ring finger on the left hand, breaking the finger when the cord pulled tight.
