Employee was stacking paper bags filled with 27 lbs of Pelite onto a pallet. While doing this he pulled muscles in his back and the ER doctor diagnosed him with muscular back pain and prescribed light duties and physical therapy for at least until his next doctors appointment which is on 9-17-09

After Locking, tagging, and testing out equipment emplyee proceeded to adjust misformed bags on the unit 2 paletizer slip plates. After completing this task he was getting out of position and caught the top of his left had on a sharp object that was out of eye sight causing a laceration.
employee was tarping the reliance trailer when the double jointed tarp rod mechanism slipped out of his left hand and spun back in his right hand. This motion caused the rod to get bent out of shape and twisted his right arm into an awkard position causing him to strain/sprain/bruise right elbow.
employee was removing a stripped bolt from a former on the palletizer when he felt his back pop.
While rolling a metal rolling pin over ore, the employee felt pain in her right elbow. The doctor diagnosed her as having "Tennis Elbow" and prescribed Physical Therapy and Naproxin. No work restrictions were given
The employee was outside moving small pieces of pipe from the ground to a pallet and used improper lifting techniques; twisting at the waist and not moving his feet in the direction of the movement caused a lower lumbar strain.
While placing a bag on the left hand packer spout, the bag that was on the right hand spout broke on the end that was facing the employee and he got DE in his right eye which caused a corneal abrasion. He was given a prescription (Ciprofloxacin) to prevent an eye infection.
When the employee went to place a 50 LB bag of product on the conveyor belt, he had to stop his forward progress abruptly because the belt had stopped and it was full. He suffered a shoulder sprain and was given a 5 LB lifting restriction.
Employee was restacking a pallet with another employee when he twisted his knee.
Employee was to change bags in our A15 Baghouse. While taking hold of cage being handed to him, the injured got the cage too close to right earlobe. His earring became caught, when he released the cage to place in position it tore his earring out of earlobe. Medical Treatment Only Accident. Stitches/Sutures to reattach earlobe. NO Lost Time
While reaching around a guard, the employee's right hand touched the conveyor belt and was pulled between the conveyor belt and the air slide table. He received deep abrasions. The employee's post accident drug scan was positive for amphetamine and methamphetamine and he was terminated immediately.
Employee was helping another employee manually turn the v-belts so the bearings could be adjusted. He got his little finger on his left hand pulled between the sheave and the v-belts, which amputated his little finger at the outermost joint, just below the fingernail.
Packing 25 lb bags all day/went home with left wrist hurting.Then middle of palm started hurting & could not bend the middle finger on L. hand. Diagnosis is trigger hands tendonitis. EE was given injections & work restrictions of no lifting more 20 lbs with L hand. She did not notify EP until 6/08/06. She was able to work their regular job until the dr put her on rest.
While filling a 50 lb bag of diatomaceous earth, the top seam of the bag ripped open and caused product to get into the employee's right eye. Object was removed by medical staff but employee was diagnosed with Keratitis of the right eys
Injured employee was filling 50 pound bags of diatomaceous earth powder then lifting & stacking onto a pallet for storage. While lifting employee felt shoulder muscle pull/strain. Reportable due to modified duty restrictions given.
Miner was repetitively twisting to lift 50 lb. bags to refeed into kiln.
EE loced out a belt feeder, opened the guard to remove large chunks of ore that had collected there. The guard is hunged and lsipped from his grasp and struck him in the head. was wearing hard hat. this injured his back.
EMPLOYEE WAS SWEEPING FLOORS AND BUMPED KNEE INTO A SHELF
Employee was moving an empty wood pallet to stack boxes on. She jerked the pallet with one arm injuring her shoulder.
Employee strained his right shoulder when he lifted a 45 lb from the floor to a chest high pallet. Doctor released him on 12-27-04 w/o restrictions or medical treatment. Went to another doctor on 5-19-04 and was given an injection to relieve pain in his shoulder
Packing and stacking powder bags of diatomaceous earth onto pallet. went to lift a empty pallet and position it for use. when temporary employee bent over at waist he could not easily raise himself into upright position.
Employee experienced pain in left foot which he feels is caused by running from wet end of mill to the control room several times. Did not report until 12-20-04.
WHILE THE EE WAS WORKING AT THE REFEED STATION, BREAKING 50# BAGS OF PRODUCT INTO THE BIN FOR THE REPROCESSING, HE WAS MOVING A BAG FROM THE PALLET TO PUT ONTO THE GRATING FOR THE REFEED BIN BUT HE DID NOT LIFT THE BAG HIGH ENOUGH AND HE STRUCK THE BACK OF HIS RIGHT HAND ON THE EDGE OF THE REFEED STATION BIN. EE IS ON RESTRICTED DUTY AT THIS TIME.
EE WAS LIFTING AND CARRYING 50 LB. BAGS OF FINISHED PRODUCT FROM THE STAGING AREA TO THE BAG CONVEYOR WHICH IS LOCATED 6-10' FROM THE CONVEYOR. EE FELT A SHARP PAIN IN HER LEFT WRIST AND DISCO NTINUED THE TASK.
EE WAS TAKING COVER OFF P4 TO CHECK WATER LEVEL AND HOT WATER COVERED BOTH ARMS.
EE WAS BENDING CONDUIT WITH A HYDRAULIC CONDUIT BENDER. EE LIFTED AND CARRIED BENDER TO RETURN IT TO ITS STORAGE AREA. DURING THIS PROCESS HE HURT HIS BACK.
EE PACKIKNG POWDER DIATOMACEOUS EARTH PRODUCT IN45# BAGS AND STACKING ON PALLET. IMPROPER LIFTING TECHNIQUE USED CAUSING TWISTING STRAIN TO OCCUR IN LEFT FOREARM. NOT LOST TIME INJURY*RESTRICT ED DUTY ONLY* FIRST AID ONLY INJURY* SUSPICIOUS REPORT OF INJURY. EE DID NOT REPORT INJURY TO SUPERVISOR OR MANAGEMENT UNTIL 8 DAYS AFTER THE FACT. EE EVEN REQUESTED OVERTIME HOURS.
PALLET IN PALLET DISPENSER WAS AT AN ANGLE, PUT HAND IN TO STRAIGHTEN, PALLET MOVED AND CAUGHT EE'S HAND. HE PULLED HIS HAND OUT AND STRUCK IT ON A METAL FRAME CAUSING A CUT AND BRUISING ON RI GHT HAND.
EE WAS RESTACKING A PALLET OF 50 LBS BAGS, HE BENT OVER TO LIFT A BAG AND HURT HIS BACK. PRESCRIPTION ISSUED.
EE AND A CO-WORKER WERE BRINGING A NEW PIECE OF EQUIPMENT INTO THE LAB. THEY WERE ATTEMPTING TO SET IT ON THE COUNTER WHEN EE PULLED A BACK MUSCLE. PRESCRIPTION ISSUED - WORKER DID NOT REPORT ACCIDENT UNTIL 1-23-03.
OUR EMPLOYEE HAD LOCKED & TAGGED THE MILLING FAN TO INDICATE HE WAS WORKING ON IT. HE WAS TO HARD SURFACE THE INSIDE OF THE FAN. HE WAS OUT OF SIGHT OF TWO EAGLE MINE EMPLOYEES ON OTHER SIDE OF THE FAN. THEY TURNED A SHEAVE TO REMOVE DRIVE SHAFT BELTS WHICH TURNED A FAN BLADE & SMASHED HIS HAND, BREAKING A BONE & BRUISING THE HAND BADLY.
EMPLOYEE WAS CUTTING OFF #3 KILN SEALS, GRABBED A PIECE OF METAL WITH LEFT HAND, CUTTING LEFT INDEX FINGER THROUGH WELDING GLOVES.

EE WAS INITIALLY INJURED ON 5/22/02 BUT DID NOT SEEK MEDICAL ATTENTION. HE WAS CARRYING PINCH VALVES UP THREE DECKS OF STAIRS TO DO MAINTENANCE WORK. EE THEN SAUGHT MEDICAL ATTENTION ON 6/19/0 2 AND WAS GIVEN A 3 WEEK LIGHT DUTY RESTRICTION.HE HAS SINCE BEEN RETURNED TO FULL DUTY AS OF 7/15/02. EE WAS ADVISED TO USE OTHER MEANS FOR LIFTING.
WHILE CLIMBING OUT OF THE PACKING CHAIR WORKER STRAINED BACK. WORKER DID NOT NOTIFY EMPLOYER UNTIL MAY 21, 2002.
