Showing all 7In the process of stacking pallets, pallets shifted. Employee was not positioned well and injured his left shoulder. Damage includes possible torn muscles and hyperextension of left elbow.
Taking liner off door of Hammermill equipment. It was wedged, so he used a hammer to beat it loose while using hands to hold the liner of the door. It popped loose, but was heavier than he thought, as this was the first time doing this task. So the weight forced his hand downward with the liner in hand between the door. Causing laceration to the left middle finger.
Lifted a chute to put on top of conveyor. (50-100lb).
Removing Calcium oxide from a bin. Dust cloud formed when it went into wheelbarrow. Some got into left eye. EE was wearing safety glasses but not goggles which were available.
